    The Health Information Management online associate degree program is accredited by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM). According to the Bureau of Labor Statistics, health information management jobs are expected to increase by 15% from 2014-2024. The median income is $37,110, (2015 statistic), but additional credentials do add to the marketability of professionals.

    The HIM program was established in 1998 as a traditional face-to-face program. In 2007-2008, the HIM program transitioned to an “online only” program to meet the needs of working students and professionals. The exception to online courses are the requirement for clinical courses completed onsite at a facility (HITT 2261 for the associate degree and HITT 1167 for Medical Coding Specialist certificate).

    Many students admitted into the program have already completed one degree, and some have bachelors or masters degrees. The reason they pursue the associate degree training is to be eligible for the Registered Health Information Technician (RHIT) upon graduation. The RHIT credential, and the RHIA credential for the bachelor’s degree, continue to be a preferred credential that healthcare employers seek when hiring Health Information Management candidates.

    Online Technology Requirements

    Students must be self-motivated, able to comprehend information in written form, and have the ability to communicate effectively both in written and verbal forms. Students should also be able to research websites, upload and download documents, post discussions, and access email. The courses are not self-paced, but have scheduled weekly due dates for assignments, discussions, and tests. Students should expect to spend 6 9 hours per 3 hour credit course to complete assignments, readings, and tests. Instructors are available to answer questions online or by phone related to course topics.

    Skills Needed for Health Information Profession:

    ability to analyze patient data for payment or registry information,
    able to sit for long periods to complete work at work station
    research and review patient information for regulatory compliance,
    code and categorize patient information using standard coding systems.
    communicate effectively with patients, physicians, and other customers
    ability to work under pressure to meet deadlines
    keyboarding skills and ability to use software systems, patient portals, etc.

    Students may apply to the program in the semester they are completing the prerequisites. Final grades will be verified before admittance. Students are accepted prior to each semester (Summer, Fall, and Spring). Most of the students in the programs are considered part-time; taking 2 to 3 classes each semester since most have part-time or full-time jobs. There are approximately 70-80 students taking HIM classes or prerequisite courses to gain admission to the program.

    Clinical Requirement Onsite:

    Students that live outside the local Midland area are able to locate clinical sites in their area with the assistance of the HIM program. More information and directions are given to students upon acceptance into the program (courses related to associate degree and Medical Coding Specialist certificate). Prior to the clinical course, students are required to meet certain facility requirements to attend clinical practice.     Course 5: Medical Billing Insurance Claims Process

    The medical billing insurance claims process starts when a healthcare provider treats a patient and sends a bill of services provided to a designated payer, which is usually a health insurance company. The payer then evaluates the claim based on a number of factors, determining which, if any, services it will reimburse.

    Let’s briefly review the steps of the medical billing procedure leading up to the transmission of an insurance claim. When a patient receives services from a licensed provider, these services are recorded and assigned appropriate codes by the medical coder. ICD codes are used for diagnoses, while CPT codes are used for various treatments. The summary of services, communicated through these code sets, make up the bill. Patient demographic data and insurance information are added to the bill, and the claim is ready to be processed.

    Processing Claims

    A number of technical protocols and industry standards must be met for insurance claims to be delivered expediently and accurately between medical practice and payer.

    Medical billing specialists typically use software to record patient data, prepare claims, and submit them to the appropriate party, but there isn’t a universal software application that all healthcare providers and insurance companies use. Even so, insurance claims software use a set of standards, mandated as by the HIPAA Transactions and Code Set Rule (TCS). Adopted in 2003, the TCS is defined by the Accredited Standards Committee (ACS X12), which is a body tasked with standardizing electronic information exchanges in the healthcare industry.

    There are two different methods used to deliver insurance claims to the payer: manually (on paper) and electronically. The majority of healthcare providers and insurance companies prefer electronic claim systems. They are faster, more accurate, and are cheaper to process (electronic systems save around $3 per claim). But because paper claims have not yet been completely removed from the insurance claims process, it is important for the medical biller and coder to be well versed with both electronic and hardcopy claims.

    Filing Electronic Claims

    Certain technologies have been introduced into the system in order to expedite claim processing and increase accuracy.

    Some healthcare providers use software to electronically enter information into CMS-1500 and UB-04 documents. Using “fill and print” software eliminates the possibility for unreadable information. This software may also include certain types of “scrubbing,” or tools that check for errors in the documents. While these tools do decrease the amount of errors made in filling out claim forms, they are not always 100 percent accurate, so medical billers should remain diligent when filling out forms using software.

    Optical Character Recognition (OCR)

    OCR equipment scans official documents, electronically isolating and recording information provided in the different fields, and transferring (or auto-filling) that information into other documents when necessary. While OCR technology helps make hardcopy claim processing much more efficient, human oversight is still needed to ensure accuracy. For instance, if the OCR miscalculates a simple digit in a medical code, that error must be flagged and manually corrected by a medical billing specialist.

    Note that when OCR equipment is not available, it is possible for a medical billing specialist to manually convert CMS-1500 and UB-04 documents into digital form using conversion tools called “crosswalks” (note that the same term applies for tools used to convert ICD-9-CM codes to ICD-10-CM). You can find crosswalk references from a number of different sources.

    Filing Manual Claims

    Paper claims must be printed out, completed by hand, and physically mailed to payers. The healthcare industry uses two forms to submit claims manually. Since processing paper claims requires more manual interaction with forms and data, the opportunity for human error increases compared to electronic claims. Documents can be printed improperly, and handwritten codes can be incorrect or illegible. The forms can also be mailed to the wrong address, with insufficient postage, or disrupted by logistical complications with the delivery services. These errors are costly for the healthcare provider, often resulting in form resubmission (a time-consuming process) and payment delays.

    Generally, healthcare professionals like family physicians use form CMS-1500, while hospitals and other “facility” providers use the UB-04 form.

    The CMS-1500 is the universal claim form used by non-institutional healthcare providers (private practices, etc.) to bill Medicare for Part B covered services and some Medicaid-covered services, and is accepted by most health insurance providers. The CMS-1500 is maintained by the National Uniform Claim Committee (NUCC), and was previously updated to include National Provider Identifiers (NPIs), or unique numbers required by the Health Insurance Portability and Accountability Act (HIPAA).

    Form CMS-1500 contains all the basic information needed to submit an accurate claim. This includes fields for the patient’s demographic information, insurance information, and boxes in which to provide medical codes and corresponding dates of service. Certain boxes are used exclusively for Medicare and/or Medicaid. It is important to note that different payers may provide different instructions on how to complete certain item numbers. The medical biller and coder should be familiar with specific payer requirements before filling out the form.

    Form UB-04, also maintained by the NUCC, is very similar to the CMS-1500, but it is used by institutional healthcare providers, such as hospitals. Like the CMS-1500, the UB-04 is used in lieu of electronic claims when the facility meets any number of exceptions granted by the ASCA. It is the responsibility of the facility to self-assess whether these designated exceptions apply to their operation, granting usage of manual claims. Also similar to the CMS-1500, certain payers may not require all fields, or data elements, to be completed.

    The role of clearinghouses

    Once a file is created using these standards, it is usually sent off to a clearinghouse. The clearinghouse is a third-party operation that primarily acts as a middleman between healthcare providers and insurance carriers.

    Think of the clearinghouse as a central hub, or a single location where all claims are sent to be sorted and directed onward to all the various insurance carriers. Typically, clearinghouses use internal software to receive claims from healthcare providers, scrub them for errors, format them correctly in accordance with HIPAA and insurance standards, and send them to the appropriate parties. Clearinghouses generally keep medical practices in the loop during this process by providing reports on the status of claims.

    This third party is necessary because healthcare providers typically have to send high quantities of insurance claims each day to a variety of different insurance providers. Each of these insurance providers may have their own submission standards. If a medical practice’s billing staff was solely responsible for transmitting insurance claims under both insurance and HIPAA requirements, the potential for error would increase dramatically, not to mention the time required for formatting each claim to specific insurance carrier.

    When choosing a clearinghouse, a healthcare provider should consider two main factors:

    • Does the clearinghouse have the capability to work with the insurance providers the practice works with most often?
    • Can the clearinghouse accommodate claims transmissions from the insurance provider’s practice management software?

    Confirming these questions ensures that all transmissions run smoothly.

    Wrapping Up Course 5

    Healthcare providers prepare insurance claims using information provided in the patient’s bill. Occasionally, the claim is prepared manually and sent by mail. In most cases, the claim is sent electronically (having either been prepared using claim software or scanned from a hard copy) to a clearinghouse. The clearinghouse checks the claim for errors, formats it according to HIPAA and insurance guidelines, then transmits it to the appropriate payer, while also sending a report back to the healthcare provider.

    After the claim has been evaluated, the insurer must provide both the patient and healthcare provider with an Explanation of Benefits (EOB). The EOB breaks down the adjudication process, showing the dates of service, procedures and charges, patient financial responsibility, and the amount paid to the healthcare provider. At this point, the health insurer sends payment to the healthcare provider, usually in the form of an electronic fund transfer.

    The insurance claims process can be complex. Fortunately, there are tools to help complete insurance claims on a day-to-day basis. Medical billing professionals who are familiar with these tools and all documents (both paper and electronic), industry standards, individual insurance company regulations, clearinghouse procedures, and the adjudication process will be prepared to succeed.

    The American Academy of Professional Coders found in its 2011 National Salary Survey that 44% of certified medical coders in the medical billing profession earn between $35,000 and $50,000 per year. These figures are matched by the findings of the U.S. Bureau of Labor Statistics, which classifies medical biller and medical coders as Health Information Technicians.

    With the dynamic changes in healthcare that are occurring every day, the demand for medical billers and medical coders is expected only to increase. In October 2013, a new diagnosis and procedure coding system will be put in place across the industry, providing thousands of new codes used to describe medical conditions and treatments. As the industry prepares for the implementation of ICD-10. well-trained professionals are earning the respect and salary they deserve.

    Medical coding and medical billing training requires a thorough understanding of medical terminology, anatomy and physiology, and the art and science of medicine. This understanding is coupled with a background in the business of medicine. Medical coders and medical billers deal adhere to professional guidelines established by the U.S. Department of Health and Human Services, the American Medical Association, professional specialty organizations, the federal Centers for Medicare and Medicaid Services, and contractual obligations with private third party payers.

    Entry level positions often require formal training to ensure a baseline understanding of the complexities that make up the medical field. The Bureau of Labor Statistics found in September 2010 that a total of 176,090 people were employed in medical billing and coding. Of those, the majority were employed in inpatient hospitals. Inpatient medical coding follows different procedures than outpatient medical billing. Hospital coders often have a degree in Health Information Management, which encompasses medical record technology and maintenance, as well as medical coding and billing. Due to the complexity of inpatient billing administration, inpatient hospital coders usually earn above the mean income of coders in other situations.

    The Bureau of Labor Statistics found that 41,450 medical billers were employed in physician offices. A physician medical practice can range from one doctor treating a small number of patients, to a large, cross-specialty practice made up of many doctors, nurse practitioners, assistants, therapists, technicians, and support staff. The billing office is where all the services provided by these practices are translated into industry-standard code, and the information is transmitted to insurance companies and government entities for reimbursement.

    Because of the wide disparity between physician office sizes and resources, the average salary of $29,030 may seem low, but it accounts for small, general practices in rural areas, as well as large practices in metropolitan markets. Anecdotal evidence shows that the medical biller is considered one of the top tier professionals within the physician office setting.

    Medical coders and medical billers are also employed in nursing care facilities, short-term rehabilitative hospitals, outpatient surgery centers, physical therapy clinics, nutrition clinics, and charitable, non-profit healthcare clinics. Wherever medical services are performed, those services are translated into code. Healthcare is a growing and dynamic industry that affects millions of lives every day.

    The Centers for Medicare and Medicaid Services projects that in 2020, $46 billion will be spent on national healthcare. The vast majority of this money will be spent by third party payers, with the bills being submitted in code. The economics of healthcare delivery and accessibility relies on properly trained medical coders and medical billers who have a solid foundation on the precepts and principles of Common Procedural Terminology (CPT codes). the International Classification of Diseases (ICD-9 and, in October 2013, ICD-10 ), and the Healthcare Common Procedure Coding System (HCPCS).

    Knowing how these various coding methodologies are used to gain appropriate and justifiable reimbursement for medically necessary services is the specialty of medical coders and medical billers. The right education will provide the foundation of a successful career in a field that changes on a regular basis. With ongoing government mandates beyond those publicized in the media, the employment opportunities for skilled medical billers and coders can only increase. In January 2012, the entire industry converted its format for electronically submitted healthcare claims. While most news media did not cover the 5010 conversion, medical billers and coders made the transition smoothly due to their training and adaptability.

    As the healthcare industry changes, trained, professional medical billers and medical coders will meet the challenges, keeping the healthcare industry running and providing necessary services to those who need it.

    There are some free “Mini Courses” online that are simply a very generalized document about medical billing and coding. It’s usually a document a few pages long or a short video presentation that gives a sample of their material. The hope is that you’ll be impressed enough to commit to purchasing the entire training program.

    Be careful – I fell for this and spent several hundred dollars. It was an impressive looking website and slick marketing. Once I purchased all I received was a notebook of general information I could have easily found on the internet. Boy did I feel like a fool! And forget trying to get a refund.

    Look out for Scams – Hold On to Your Credit Card
    Also be wary of free medical billing training trial programs that require you to enter credit card information before viewing their “free” material. Supposedly if you decide not to take the complete course you have to call to cancel. I’m suspicious of anybody that wants a credit card up front. Once they get your credit card number it can be very difficult to get a refund or prevent them from charging you.

    I’ve done this with other “free” offers before and found it almost impossible to cancel and get them to stop charging my credit card.

    There are ways you can get free medical billing training. However the quality of the training may be lacking.

    Get Real World Experience
    Offer to work part time for a medical billing service for free – for a limited time of course. When I was anxious to learn medical billing, I offered to work for free. My thinking was that is a great way to get hands-on real world training.

    I had a friend who started a medical billing business and was very busy. I talked to her about how she got started and asked for her advice. During our conversations I offered to work for free for a short time until I learned enough to start my own business.

    Medical billing is the sort of work that can be done at home easily. Fortunately they didn’t take me up on the free part but did pay me for part time work from home. To me that was like getting paid to learn!

    I started out working on insurance aging which involved calling insurance companies to follow up on unpaid claims and determine why they were rejected. I got progressively more experience and responsibilities until I felt comfortable marketing my own business.

    If you are starting a medical billing business, this type of work is an excellent way gain experience and cultivate contacts with other people in the field – such as doctors, office managers, and other billers and coder.

    There is a website called Quizlet which the worlds largest online student and teacher community. Their mission is to help students master the subjects they are learning.     Get Paid to Learn
    Even better than free medical billing training is the thought of getting paid to learn. Like I mentioned above, any environment you can get into – even for pay that is less than the going rate – that allows you some experience in the field is a great help toward getting trained. This would be short term of course.

    Consider opportunities at a medical billing services company, hospital or doctors office, or insurance company. Anything that gets you exposed to the field looks good on your resume. Remember your goal is getting experience – so a short term sacrifice in pay may be worth it to prove yourself and get good references.

    One of the requirements I notice in medical billing job postings is experience in the field – probably more than training or credentials for medical billing. That’s why getting any experience you can is important. In this field there are many things that can’t be taught in a classroom environment.

    Update 2017 – VideoTrainingPro.com used to offer very good free medical billing training video’s. However they have recently removed them from their site. They seem to be concentrating on just Window’s training now. Not sure why as they were very well done.

    If we can find where they have been moved to we will post a new link. They had an extensive (113) catalog of video’s on just about anything a medical billing specialist does.

    Research the Internet for Free Medical Billing Training
    There’s so much information from credible sources available on the internet on medical billing. You can educate yourself and find information on just about any topic. The challenge is being able to separate the credible and knowledgeable from the pretenders.

    Even on this site I try to document my experiences, information, and other free medical billing training resources that may be helpful to others getting started in the medical billing and coding profession.

    I found it can be difficult to find any in-depth information on particular topics. This takes a lot of time. There is a lot of online information on medical billing that seems to be almost entirely dedicated to marketing and not educating. These sites are just trying to sell something. Or on the opposite end of the spectrum having so much information that it’s overwhelming.

    Industry Topics
    An example of this would be the HIPAA. There’s a tremendous amount of information about HIPAA available at CMS – The Centers for Medicare and Medicaid Services. But trying to determine what is relevant and applicable to a small practice is almost overwhelming as there are so many facets of HIPAA.

    A lot of billing services are smart to educate themselves well on HIPAA (and other industry issues) and use this to educate, and market to, small practices. This is a great way to open doors and establish yourself as an expert.

    American Association of Professional Coders ( AAPC ) is also has some great info on medical billing – although their main concentration is coding.

    Something else that would have been helpful to me when getting into medical billing was something that explained how an insurance claim is processed.

    The Medical Billing Insurance Claim Flowchart is another free medical billing training tool that shows a very simple flowchart explaining how healthcare claim processing works and how insurance claims billing works.
