Medical Bills Are the Biggest Cause of US Bankruptcies: Study

Bankruptcies resulting from unpaid medical bills will affect nearly 2 million people this year—making health care the No. 1 cause of such filings, and outpacing bankruptcies due to credit-card bills or unpaid mortgages, according to new data. And even having health insurance doesn’t buffer consumers against financial hardship.

The findings are from NerdWallet Health, a division of the price-comparison website. It analyzed data from the U.S. Census, Centers for Disease Control, the federal court system and the Commonwealth Fund, a private foundation that promotes access, quality and efficiency in the health-care system.

“A lot of Americans are struggling with medical bills,” said NerdWallet Health Vice President Christina LaMontagne.

NerdWallet estimates that households containing 1.7 million people will file for bankruptcy protection this year.

Even outside of bankruptcy, about 56 million adults—more than 20 percent of the population between the ages of 19 and 64—will still struggle with health-care-related bills this year, according to NerdWallet Health.

And if you think only Americans without health insurance face financial troubles, think again. NerdWallet estimates nearly 10 million adults with year-round health-insurance coverage will still accumulate medical bills that they can’t pay off this year.

High-deductible insurance plans requiring consumers to pay more out-of-pocket costs are a challenge for many households.

“With an average American family bringing home $50,000 in income, a high medical bill and a high-deductible insurance plan can quickly become something they are unable to pay,” LaMontagne said. “If you have an out-of-pocket maximum of $5,000 or $10,000, that’s really tough,” he said.

The analysis of rising health costs is the first of its kind for NerdWallet.

Obamacare ‘Not a Panacea’

With millions buried under medical bills, more insured under the Affordable Care Act will not completely solve that problem, LaMontagne said. While the ACA’s reforms will indeed give more people coverage, NerdWallet’s data shows that millions of people with year-round, full coverage are still overwhelmed by medical bills, she said.

“I don’t think Obamacare is going to get rid of the situation,” LaMontagne said. “The data suggests that already-insured Americans are struggling. With the expansion of insurance, it doesn’t seem like that problem will go away entirely. It’s not a panacea.”

The number of medically related bankruptcies is slightly less than the rate of recent years. Despite the anticipated 2013 dip, such bankruptcies represent about three out of every five filings.

“A lot of Americans probably think about bankruptcy as coming from unpaid credit-card debt or mortgages,” LaMontagne said. “But the root cause of all those troubles may well be medical bills.”

Not surprisingly, more than 11 million people will take on additional credit-card debt to cover mounting medical bills, LaMontagne said. Because credit cards often charge high interest rates for unpaid balances, debt only mounts, creating a vicious cycle for consumers.

Meanwhile, NerdWallet found, 1 5 million people will deplete their savings to cover medical bills. Another 10 million will be unable to pay for necessities such as rent, food and utilities because of those bills.

Skipping Prescription Medications

When their savings are gone and their credit cards are maxed out, stretched consumers take the drastic step of cutting back on prescription medications.

More than 25 million people are skipping doses, taking less medication or delaying refilling prescriptions to save money, NerdWallet found.

“That statistic is actually quite troubling,” LaMontagne said. Delaying needed medication is a short-term fix that only triggers more health problems in the long term, she said.

Analysis of 2016 Premium Changes in the Affordable Care Act’s Health

Updated Oct. 27, 2015 to include final rates for CA, ID, and NY, and an update to NM rates.

The table below presents an update to our previous analysis of 2016 changes in premiums for the second-lowest cost (“benchmark”) silver marketplace plans in major cities in all 50 states and the District of Columbia.

Among these major cities, the percent change from last year in the benchmark premium ranges from -10.6% in Seattle, Washington to 38.4% in Nashville, TN. The simple average of these rate changes is 10.1% before accounting for the premium tax credit.

For a 40 year old making $30,000 per year, the average change after tax credits would be -0.2% (holding age and income constant). 2016 benchmark silver rates for a 40 year old will range from $183 per month in Albuquerque, NM to $719 per month in Anchorage, Alaska before accounting for the tax credit. If this 40 year old makes $30,000 per year, the premium paid by the consumer after the tax credit would range from $163 per month in Anchorage, Alaska to $206 per month in most of the country.

We added a new row showing the average premium change weighed by the 2015 effectuated enrollment for the state in which the city is located. Enrollment data are not available for all counties or cities. The weighted average increase is 3.6% across these cities, before accounting for the premium tax credit.

For comparison, these cities saw an average change of -0.3% in the benchmark premium from 2014 – 2015 (the weighted average decrease was also -0.3%) before accounting for the tax credit.

Here’s How Much Your Birth Control May Cost Under AHCA

The American Health Care Act. the GOP health care plan that aims to repeal and replace Obamacare, has narrowly passed in the House, and currently awaits a vote in the Senate.

The controversial bill has been met with considerable pushback, as it would ultimately leave 24 million Americans without health insurance by 2026, according to an analysis from the Congressional Budget Office.

Should the legislation pass, women are poised to face especially complex challenges when it comes to accessing birth control, which has been covered by Obamacare thanks to the “birth control benefit .”

The Affordable Care Act requires insurance plans to cover all Food and Drug Administration (FDA)-approved methods of birth control for women without having to pay any out-of-pocket costs.

Under the AHCA, however, women would no longer be able to reap any of those benefits.

While Republicans may view a woman’s access to birth control as a “nitty-gritty detail ” in crafting health care legislation, any woman of child-bearing age can assure you the issue is far from trivial.

Let’s talk about how much each contraceptive method actually costs when you pay out-of-pocket.

While the prices vary depending on the circumstances, for most brands, a pack of birth control pills (which typically provides a month’s worth of contraception) can cost as much as $50 .

However, it’s important to remember that only 17 percent of women who use a form of birth control are taking the pill.

The intrauterine device, or IUD, is another commonly used method, though it demands a much steeper price out-of-pocket.

Popular IUD brands like Mirena and Kyleena can cost anywhere between $500 and over $800 .

Similarly, a birth control implant can set you back $800 without insurance .

Unfortunately, the prices aren’t much kinder on your wallet as you make your way through the rest of the viable birth control options. Without insurance, you could pay up to $250 for the Depo-Provera shot. and $80 a month for the Nuvaring .

Mind you, these costs don’t even factor in the doctors’ visits to obtain these prescriptions, nor the follow-up visits, all of which do not come free without insurance.

So, what about Planned Parenthood?

Fortunately, clinics like Planned Parenthood provide amazing resources to women seeking forms of contraception but lack the health insurance to pay for it.

Not so fortunately, the latest version of AHCA plans to nix federal funding of the organization and bar patients from using any Medicaid dollars to access treatment at Planned Parenthood clinics.

To put this in perspective, in 2015, approximately 43 percent of Planned Parenthood’s budget. or around $500 million, came from federal grants and reimbursements, according to NPR.

Moreover, 60 percent of Planned Parenthood’s federal funding comes from Medicaid and Title X reimbursements for preventative and primary care, which includes not only birth control, but cancer screenings. as well.

Even if the AHCA manages to garner enough votes in the Senate, this does not mean Planned Parenthood will vanish overnight.

But they will lose a huge chunk of their funds, and we as patients could see an extremely alarming drop in the quality of their services.

Plus, according to the Government Accountability Office, approximately 390,000 women could lose access to preventative care. and as many as 650,000 women would receive reduced preventative care.

Cecile Richards, president of the Planned Parenthood Federation of America, discussed the potential impact of the GOP health care plan in the following statement:

This is the worst bill for women’s health in a generation. This disastrous legislation once again makes being a woman a pre-existing condition; ‘defunds’ Planned Parenthood; guts maternity coverage; strips 24 million of their health insurance; lets insurance companies charge people with pre-existing conditions exorbitant rates; forces new moms back to work shortly after giving birth; and reduces access to contraception.

In short, this bill makes it harder to prevent unintended pregnancy, harder to have a healthy pregnancy, and harder to raise a family. Alarm bells should be ringing in every house across America.

Richards urges the Senate to “ listen to the American people and reject the worst bill for women’s health in a generation.”

While it’s up to your health care provider whether or not to cover birth control, and some states, like New York. have made a plan to continue making coverage mandatory, the Trump administration has made it painfully clear that women’s health is at the bottom of their priority list.

Uh-oh. You woke up with a burning sore throat, and your mom says you need to go have it checked. But before the doctor even makes you say aaahhh, there’s probably someone else you’ll see first: the nurse.

Nurses are important people. Not only are they often the first health care professional that a sick or injured person sees, but they do their job in all kinds of settings from local hospitals to faraway military bases.

Some even work in the sky or at sea, helping to transport sick people on planes or caring for passengers on ships. In fact, anywhere in the world you can find someone who needs health care, you can probably find a nurse.

Where Do Nurses Work?

Where are you most likely to meet a nurse?

At your doctor’s office. Nurses in medical offices typically assist the doctor by asking you about your symptoms, taking your temperature and blood pressure, checking your weight, giving shots, and collecting blood or urine samples for lab tests.

Doctor’s office nurses can teach patients about their medical problems and how to take care of themselves.

You’ll also find nurses:

  • At your school: School nurses certainly see their share of kids with stomachaches, chickenpox, nosebleeds, and bumps and bruises. But they also have the important job of promoting good health. How do they do it? School nurses do vision, hearing, and scoliosis screenings and teach kids how to take care of themselves through good nutrition, exercise, and hygiene.   • At the hospital: Hospital nurses provide round-the-clock patient care. Some nurses even work in the operating rooms alongside the doctors.
  • In your neighborhood: Some nurses work directly in the community. These nurses teach people with special health problems like diabetes or asthma how to manage their conditions. They also might travel to a person’s home to provide care for those who are too elderly or sick to leave.

Whatever type of work they do, the best nurses have one thing in common: they love interacting with people. That’s because nurses do more than provide hands-on health care. They also give patients and their families compassionate support at a time when they need it most. Nurses are educated to care for the whole person, not just treat whatever health issue somebody has.

How Does Someone Become a Nurse?

It’s easy to think all nurses are pretty much the same, but that couldn’t be further from the truth. There are actually dozens of different types of nurses, each with its own level of training. Basically, the more formal education a nurse has, the more responsibilities he or she can take on.

People who want to become nurses have to decide which path is right for them. Some people know early on that they want to be a nurse. They might volunteer in hospitals when they are in high school and then go on to college to get a nursing degree. Things an LPN might do include helping a patient bathe and dress, administering certain types of medications, changing wound dressings, and taking vital signs. An LPN has at least 1 year of training in providing this kind of care.

Registered Nurse (RN): Registered nurses give medication, perform basic procedures, and work closely with doctors to monitor a patient’s condition. All RNs have a degree from a 2- to 4-year nursing program. They study subjects like chemistry, biology, anatomy, and psychology, and they also get lots of hands-on practice called “clinical training.” RNs are the most common type of nurse and most RNs have a bachelor’s degree.

Registered nurses need a nursing license. To get a license, a nurse must successfully complete a nursing program and pass a test called a licensing examination. To keep that license, a registered nurse must continue to take classes every few years to make sure his or her skills are up to date.

RNs also need additional training if they want to “specialize” that is, focus their care on one type of patient, such as newborn babies or the elderly. Registered nurses also can become certified in a certain area of expertise, such as emergency or intensive care.

Advanced Practice Nurses (APN): Advanced practice nurses are registered nurses who have gone on to get further training, including a master’s degree. There are many types of APNs, including a certified nurse midwife (who is trained to deliver babies), certified registered nurse anesthetist (CRNA, who specializes in giving and monitoring anesthesia), and a nurse practitioner (NP).

A nurse practitioner is a type of advanced practice nurse with training in a specific area, such as pediatrics. An NP often is the one who takes a medical history, does an initial physical exam, writes prescriptions, and treats illnesses and injuries. In fact, you may see an NP instead of a doctor at some of your office visits.

Nursing Today

If you’ve ever seen an old movie with nurses in it, you’ve probably noticed that nurses used to look a lot different from how they look now. They wore white dresses, white shoes, and a crisp white hat. And they were all women.

A lot has certainly changed in the last 40 years. Today you’re more likely to find nurses wearing comfortable scrubs with clogs or sneakers, and you’re also more likely to find that many are men. In fact, the number of men in the profession has been steadily rising.

Still, with people living longer and needing more health care, there are simply not enough nurses to go around. Some experts estimate that by the year 2020, there will be a shortage of 1 million nurses and that translates to a lot of sick people who may not get the care they need.

myopia (mi-OH-pee-uh), nearsightedness.Refractive error. Focusing defect created by an overpowered eye, one that has too much optical power for its length. Light rays scoming from a distant object are brought to focus before reaching the retina. Nearsighted people see close-up objects clearly but distance vision is blurry. Corrected with a minus lens (spectacle or contact) to weaken the eye optically and permit clear distance vision.

MYOPIA (Nearsightedness)

When you are nearsighted (myopic), your vision is clear for objects close to your eyes, but blurred for everything in the distance. If a child cannot see the blackboard clearly from the back of the classroom, chances are that he or she is nearsighted.

Myopia comes in all degrees, from minimal to extreme. The more myopic you are, the more blurred your distance vision, but the closer up you can see clearly. In other words, your range of clear vision is much closer to your eyes than if you weren’t nearsighted. Approximately 40% of the population has some myopia or will develop it at some time in their lives.

Most commonly, myopia begins to appear gradually between the ages of 8 and 12, though it can exist at birth or start to develop as late as age 80. Myopia may be a nuisance but it is certainly not a disease; most nearsighted people have perfectly healthy eyes. Many “myopes” are happy to be able to see things clearly up close without glasses. In fact, this ability can be a real advantage, especially after middle age.

The only symptom of myopia is blurred vision for distant objects. Eye fatigue, burning eyes, headache, and limited tolerance for reading occasionally accompany the myopia, but they are not symptoms of the myopia itself. When young children hold everything close to their face or sit very close to the television, this does not necessarily mean they are nearsighted. It may only mean that they like the way things look up close.

Sharp vision — like the picture from a properly focused camera — depends on light rays coming to a focus on the retina (at the back of the eye), just as light focuses on the film (at the back of a camera). When light rays do not come to a focus on the retina, vision will be blurred, and we say that a refractive (optical) error exists.

Myopia is just one type of refractive error. The others are hyperopia (farsightedness), astigmatism (uneven focusing), and presbyopia (the inability to focus up close, which affects most people around middle age). In myopia, the rays from distant objects focus in front of, rather than on, the retina. Myopia is like a camera that is in focus only for near objects; anything in the distance is out of focus.

What Causes Myopia?

In most cases, myopia is the result of a size variable, like foot size or tallness. The myopic eye is larger or elongated – too long for its optical power – which means its optical power too strong for the eye. (You shouldn’t think of nearsightedness as “weak eyes.”) Research suggests that ordinary myopia and how fast it progresses during adolescence are determined by heredity — it tends to run in families. It is not caused by using your eyes”too much” (you never hurt your eyes by using them).

Some populations (the Inuit eskimos, for example) have shown a statistical shift toward myopia when, over many years, some members of their society changed from outdoor activity to working inside, doing much closer work. This fact, however, does not mean that if an individual does a lot of close work, he or she will become myopic.

Aside from heredity, there are a few, less common causes. Myopia that appears (or increases) in middle age may be a sign of a beginning cataract. In uncontrolled diabetes, myopia my occur suddenly and then change erratically from day to day. And rarely, a teenager may develop myopia from keratoconus, an unusual condition in which the cornea becomes cone-shaped.

Why Does Myopia Get “Worse” as the Child Gets Older?

As children’s bodies grow, so do the eyes, which may cause a gradual increase in myopia. And just as bodily growth can be in uneven spurts, the changes in myopia may be similarly uneven. During adolescence, the change can be rather rapid and require a new, thicker eyeglass correction more than once a year, but when body growth slows or stops (usually by age 18), the myopia tends to stabilize. There is normally no reason to worry about the frequent changes in lens correction that occur during adolescence. Almost never is there any real danger to eyesight, and vision can almost always be corrected to 20/20 or better with eyeglasses or contact lenses.

Note: There is an extremely rare type of myopia, called malignant progressive myopia, which is a serious condition and leads to gradual structural damage to the eye. This type needs regular clinical evaluation. It is not related to ordinary myopia and does not develop from ordinary myopia.

Lens Correction for Myopia

Fortunately, eyeglasses or contact lenses provide a simple, effective way to provide clear vision – by optically reducing the excess power of the myopic eye. Wearing your correction will make your distance vision clear. The more nearsighted you are, the more you will want to wear your correction. Not wearing it, however, will not harm your eyes in any way.

Nearsighted children should be checked for glasses every year or so, and nearsighted adults every 2 to 3 years — more frequently, of course, if you start having any symptoms that seem to be related to your eyes. For eye safety, impact resistant lenses are required by law for all eyeglasses, but the safest ones for active people are made of polycarbonate plastic. These offer the best possible protection against eye injuries.

Other Methods of Correction

There is some evidence that special contact lenses (in a procedure called orthokeratology), bifocal eyeglasses, or dilating eyedrops can be used to slow the progression of myopia, but the effects are very minimal and temporary, and rarely worth the extra effort and cost. Treated or not, myopia almost always advances to a certain point and then stops changing.

Refractive surgery is being well publicized as the modern way to reduce your dependence on glasses or contacts. This type of surgery includes several procedures for permanently lessening or possibly even eliminating myopia. Some involve using an excimer laser to reshape the cornea (PRK, LASIK); one uses a surgically implanted plastic corneal ring (the effect is reversible if the ring is removed). All are intended to reduce the cornea’s optical power to achieve normal or near-normal focus. These procedures are not appropriate for everyone, and are not done on a young eye that is still growing. Before making a decision to have refractive surgery, you should learn all you can about it and the different procedures that are available.

Course Requirements for a Health Administration Associate Degree

The specific courses you will take will depend on what degree you are pursuing and the area in which you plan to concentrate. Most programs require certain core courses in the degree area, such as classes on how the health care system works, technology for managing health care practices and functions, basic medical law and ethics, mathematics, medical terminology and introduction to anatomy. If you are on the arts track, you might take some courses in the humanities, English and writing, civilization, history, sociology or a foreign language. Those on a science track might enroll in introduction to biology, chemistry, psychology and even physics classes. If you are planning to concentrate in the general business aspects of the field, you may take additional courses in business and administration, organizational theory, customer service and marketing. If you seek an area of concentration, you will want to add electives in a specific field such as pharmacology or information technology. Typical degrees require about 60 credit hours of work.

Job Prospects with a Health Administration Associate Degree

Most students look for a job when they graduate, even if they plan to go on and get a bachelor’s degree. Work experience can help you get into a better school if you are continuing your education. If you plan to stay in the workforce, experience is the key to better-paying jobs. Getting an entry-level administrative job in a doctor’s office or in a hospital is a common choice, but it isn’t your only option. Insurance companies, clinics, pharmacies and long-term care facilities also look for people with health care administration credentials. You may find yourself doing a little bit of everything in a small physician’s practice. In a larger facility, you may be more focused in an area such as managing patient records, coding for insurance billing or managing inventory at a pharmacy.

The U.S. Federal Trade Commission (FTC) needs your help in halting a surge in the fraudulent marketing of medical discount cards. In these schemes, marketers typically represent that consumers will receive low-cost health insurance or medical benefits. In reality, consumers do not receive health insurance or any meaningful medical benefits. Instead, they receive a card that purports to provide discounted rates with medical providers. Consumers find that the promised discounted rates are illusory.

These bogus medical discount programs are marketed to consumers in a variety of ways, including illegal recorded telephone calls (robocalls), unsolicited faxes, radio and television ads, and web sites. Regardless of the medium, the ads often target seniors, claiming that the discount cards will supplement Medicare.

The FTC, which is the federal government’s consumer protection agency, brings federal court law enforcement actions to stop fraud and obtain refunds for consumers. If you have information about a medical discount card scheme, or learn of a victim of such a scheme, call the FTC at 1-877-382-4357 .

Robert Kann, a licensed physical therapist designed this unique program specific to Parkinson’s disease and has been recognized the University of Michigan as one of the top two Parkinson’s program in the country. Reflective Practice in Social Work — The Ethical Dimension

Recently, I received an urgent voicemail message from a hospital social worker: “Please get back to me as soon as possible. I have a meeting tomorrow morning with our head of human resources, and I’m very nervous about it.”

Later in the day, I connected with the social worker and learned the following: He had been employed by the hospital for seven years and had never been disciplined. His current predicament began when his immediate supervisor called him in to discuss concerns about possible boundary violations and an alleged inappropriate dual relationship with a hospital patient. The social worker explained to me that in his personal life he is actively involved in a community-based group of parents who adopted children from China. The group sponsors a wide range of activities to support and enhance the children’s ethnic identity. Through this involvement, the social worker said, he and his wife had become very friendly with several other adoptive parents.

About three weeks earlier, one of the parents who had become a good friend was admitted to the social worker’s hospital for treatment of a chronic, debilitating infection. The friend did not receive social work services. During the friend’s hospital stay, the social worker occasionally stopped by his room to say hello and inquire about the friend’s health. The patient’s attending physician had collaborated professionally with the social worker in other hospital cases and was well aware of the patient’s friendship with the social worker.

One afternoon during the patient’s hospital stay, the physician contacted the social worker and explained that the patient was distraught after having just learned that he was diagnosed with bone cancer. According to the social worker, the physician asked the social worker to visit the patient and offer emotional support. The social worker visited the patient in his room and spent about an hour helping his friend process the distressing medical news.

The social worker documented this patient encounter in the hospital chart. During a random quality-control review of social workers’ chart entries, the hospital’s social work supervisor read the note and became concerned because the social worker had not been assigned to provide social work services to this patient. The supervisor learned of the social worker and patient’s friendship and notified the director of human resources, who documented this “incident” in the social worker’s personnel record and asked to meet with the social worker.

The Nature of Reflective Practice
In 1983, the late scholar Donald Schon published his influential and groundbreaking book The Reflective Practitioner: How Professionals Think in Action . Schon’s thesis, based on his extensive empirical research, was that the most skilled and effective professionals have the ability to pay critical attention to the way they conduct their work at the same time that they do their work. Schon coined the terms “knowing-in-action” and “reflection-in-action,” which suggest that some professionals can take a step back and think hard about what they are doing while they are doing it. The concepts are akin to the widely used social work concept “use of self.”

Ordinarily the concepts of knowing-in-action and reflection-in-action are applied to practitioners’ cultivation and use of technical skill, whether in surgery, architecture, town planning, engineering, dentistry, or psychotherapy. In my view, and as the above case demonstrates, social workers would do well to extend the application of these compelling concepts to their identification and management of ethical issues in the profession. Ideally, effective practitioners would have the ability to recognize and address ethical issues and challenges as they arise in the immediate context of their work, not later when someone else points them out. Put another way, social workers would have a refined “ethics radar” that increases their ability to detect and respond to ethical issues.

Of course, the most important benefit is client protection. However, an important by-product is self-protection, that is, the increased likelihood that social workers will protect themselves from ethics-related complaints.

Implementing Reflective Ethics Practice
Certainly the hospital social worker who called me with panic in his voice would have benefited from reflective ethics practice and highly sensitive ethics radar. Had he reflected on the ethical dimensions of the boundary challenges that emerged when he interacted with his friend and hospital patient, it is likely that this well-meaning practitioner would have avoided his unpleasant encounter with the human resources department. The social worker’s decision to visit his friend was not the error; that was a humane and compassionate gesture. The error, rather, was not reflecting on his role in that moment and managing the boundaries carefully, including discussing them with his friend and his supervisor.

In my experience, ethics-related reflection-in-action entails three key elements.

Knowledge: Skillful management of many ethical dilemmas requires knowledge of core concepts and prevailing standards. Ethics concepts are addressed in professional literature and standards exist in several forms, including relevant codes of ethics, agency policies, statutes, and regulations. For example, the National Association of Social Workers’ Codeof Ethics includes explicit standards pertaining to boundaries, dual relationships, and conflicts of interest (especially section 1.06). It would have been best for the hospital-based social worker to consult relevant literature and standards with regard to conflicts that can arise when a social worker encounters a friend or social acquaintance in the work setting. The hospital’s personnel policies also prohibit dual relationships that involve conflicts of interest.

In some cases, although not all, statutes and regulations address ethical issues. In the United States, both federal and state laws address various ethical issues, such as confidentiality, privileged communication, informed consent, and social workers’ ethical conduct. Such laws would not have been particularly helpful in the hospital social worker’s case, but often they are helpful and critically important, for example, when social workers must decide whether to disclose confidential information without clients’ consent to protect a third party from harm or whether parental consent is necessary to provide services to minors who seek help with substance abuse but insist that this information be withheld from their parents.

Transparency: Reflective social workers who sense an ethical issue share their concern with supervisors, colleagues, and appropriate administrators. An effective way to protect clients and practitioners alike is to avoid any suggestion that the ethical issue is being handled “in the dark.” Such clarity demonstrates social workers’ good faith efforts to manage ethical dilemmas responsibly. When appropriate, clients should be included in the conversation.

Process: Although some ethical decisions are clear-cut, many are not. The hospital social worker who contacted me was unsure about the best way to manage his involvement with a good friend who had become a patient. Unfortunately, the social worker did not notify his supervisor about the dilemma or seek consultation. He documented his lengthy hospital-room encounter with the patient, but doing so in the client’s hospital chart created the impression that the social worker was functioning in his professional capacity, not as a friend. My hunch is that had the social worker notified his supervisor of his friendship with the patient and made clear that any contact with the patient occurred as a friend, the social worker may have avoided any adverse personnel issues. What I have learned is that many ethical decisions are not simple events; they require a considerable, often painstaking, process.

During the course of the profession’s history, social workers have refined the art of reflective practice. Historically, these skills have been applied primarily to clinical, policy, advocacy, and administrative functions. Clearly, reflective practice should extend to ethics as well.

The School of Health Care Administration at Oklahoma State University Center for Health Sciences trains leaders to guide our state s hospitals, clinics, nursing homes and other health organizations. The OSU-CHS master s degree in health care administration was recently ranked the fifth-best online program in the nation .

Overview

The Health Care Administration (HCA) program is a master of science degree in health care administration with options in health care leadership and entrepreneurship or administration. The curriculum provides exposure to management concepts, processes and techniques associated with administration and entrepreneurship functions in a variety of health care organizations. This degree is ideal for those individuals working in health care who wish to move into management or executive positions however healthcare experience is not required. This degree offers on-site courses in Tulsa as well as distance learning opportunities. This degree can be completed in class or fully online.

The M.S. in Health Care Administration consists of 32 total hours with a creative component or thesis including six hours of general graduate level electives.

Electronic and paper travel vouchers

(e-MCOs and MCOs)

Which type of voucher do you have?

Electronic travel vouchers (e-MCOs) are issued in the form of a letter. Within the letter is your 13-digit e-MCO number.

A paper travel voucher is also called a Miscellaneous Charges Order (MCO). Paper travel voucher numbers always start with ‘014 40’, followed by 8 other numbers.

If you have an Electronic Travel Voucher (e-MCO)

There are two ways to redeem an e-MCO.

Option 1:

For faster credit, redeem your e-MCO when you complete your booking with Air Canada Reservations or at an Air Canada staffed airport. Simply provide the e-MCO voucher number to the agent at the time of booking.

Option 2:

  • The refund will be processed to the original form of payment of the ticket.
  • Please allow ten (10) business days for your request to be processed.

Note. An e-MCO cannot be used in conjunction with a Promotion Code.

If you have a Paper Travel Voucher (MCO)

There are two ways to redeem it:

  • Option 1: Redeem your paper voucher when making your new booking directly with your travel agent or at any Air Canada staffed airport.
  • Option 2: Apply the credit for a new online booking from the comfort of your own home:
    • Book and pay for your reservation online. Then, mail the electronic ticket/receipt along with your original travel voucher (MCO) to Air Canada’s Refund Department upon completion of all travel.

    Nursing Bridge program

    Our Nursing Bridge Program is for RNs that have a bachelor’s degree from a regionally or nationally accredited college or university in an unrelated field but want to earn their master’s degree in nursing. This program helps nurses in this category more quickly earn their MSN, saving them time and money. You will apply the knowledge you gained as an undergraduate along with your hard-earned nursing knowledge towards our Master of Science in Nursing program.

    In our Nursing Bridge program you’ll take the same introductory course all MSN students take, however you’ll be required to take three nursing upper division level bridge courses. These courses provide you the additional skills and knowledge you need, including a mastery of foundational, undergraduate content in the areas of nursing theory, evidence-based practice and research and standards of professional practice.

    Required Bridge Courses

    1. Theories and Models of Nursing Practice
      In NUR403 you’ll study the behaviors, attitudes and values necessary for theory-based professional nursing practice. Concepts of professional nursing are presented within the unifying framework of Dr. Jean Watson’s “Theory of Human Caring.”
    2. Evidence-Based Nursing Research and Practice
      In NUR443 you’ll learn how to interpret, evaluate and use research in your nursing practice, as well as how to conduct your own research.
    3. Health Assessment and Promotion of Vulnerable Populations
      In NUR440 you’ll learn how to develop interviewing skills, refine the process of giving a health assessment and use preventative health interventions when working with diverse and vulnerable populations.

    Current RN license required.
    These degree programs include clinical courses or a practicum, which may not be completed in certain states. Please check with a University Enrollment Representative.

    Critical care nursing is a relatively new concept. Advancements in medical technology have made medical care more complex. To provide the best care possible, critical care nurses are provided for the neediest patients. When it comes to medical emergencies, critical care nurses are the people that patients want to see. Critical care nursing is all about providing the highest level of care to ill patients in a timely fashion.

    ICU Nurse Job Description

    An ICU nurse has the responsibility to handle patients with life-threatening illnesses. His or her job consists of working in intensive care units and emergency departments at healthcare facilities. These individuals insure the proper care of patients and their families.

    Critical care nurses are the advocate for ill patient and the family members. They provide specialized experience, knowledge, and skills that their patients need. They may have to work in environments where patients receive intense interventions and therapies, and complex assessments, and they must offer humane care for their patients.

    The responsibilities of a critical care nurse may include:

    • Providing the necessary care for patients
    • Acting as a liaison for patients and the families of patients
    • Respecting the rights and beliefs of patients
    • Monitoring the care that patients receive
    • Offering assistance that is in the best interest of the patient
    • Interceding for patients in situations that require action immediately
    • Representing the patient according to his or her choice

    How to Become an ICU Nurse

    To become an ICU nurse, it is necessary to earn a degree in nursing. The options to become a nurse are available through an associate’s degree, a diploma program, or a bachelor’s degree in nursing. Many current ICU nurses choose to advance their education by earning a master’s degree in nursing to become an advanced practice nurse so that they can increase their opportunities.

    Most hospitals have a minimum education requirement of a bachelor’s in nursing to be considered for a position. Many hospitals may consider honoring certificates from diploma programs for graduates of their own nursing program.

    Becoming an ICU nurse does not require certification, but most graduates choose to earn the credentials to increase their marketability. Registered nurses have a greater opportunity for employment and advancement than non-certified ICU nurses do. Certification is an indicator of the exceptionally high level of expertise in the field of nursing.

    For those who wish to become certified, it is necessary to have 2 years of experience working with critically ill patients. They can earn the critical care registered nurse certification by taking the CCRN exam administered by the AACN. Many other nursing associations offer the exam necessary to receive certification as an ICU nurse.

    More and more employees are requiring that ICU nurses receive the credentials as advanced practice nurses by taking a nationally approved certification examination due to the managed care and Medicare stipulations for reimbursement of funds.

    ICU Nurse Salary

    The average nursing salary for an ICU nurse is $68,111 annually. Areas of the country in which an ICU nurse can make higher wages are Phoenix, San Francisco, Tampa, and Washington D.C. An ICU nurse can make a salary range of $74,089 to $109,113 annually. The high demand for certified nurses can increase the annually salary significantly, depending on the location, education, experience, and facility.

    Critical Care Nursing Jobs

    ICU nursing jobs are in great demand around the country so the outlook is exceptional. The need for qualified registered nurses will create over 581,500 jobs by the year 2018.

    The nursing shortage is very evident in critical care nursing. The number of travel nurses and temporary nurses has skyrocketed in the past few years. The greatest number of critical care nurses is needed in pediatrics, emergency departments, adult critical care units, and neonatal units.

    Many hospitals are now offering qualified ICU nurses very attractive bonuses, continuing education stipends, and relocation bonuses. The use of web based orientation programs are now being used to prepare and attract experience, licensed nurses to become ICU nurses.     The Early Childhood Center (ECC), located on the beautiful campus of Southern Illinois University Edwardsville (SIUE), provides full-day, year round early childhood care and education for the children of students, faculty, staff, and others in the University community, ages two through five.

People who volunteer can work flexible hours – as few as two hours per week, once a month, etc.

    The Orthopedic Institute
    The Orthopedic Institute at Monongahela Valley Hospital is changing the way that patients and their families experience joint replacement. This proven program is centered around the fundamental principle of wellness and volunteers play an active role in the patient experience. Volunteers work beside the medical and professional staff as they provide care to patients. They support patients – serving-one-on-one – sometimes even as an extended family ‘coach’ during therapy and group activities when needed.     Graduate Programs in Medical Informatics

    Master’s Degree: Thesis Option

    This option requires a research thesis prepared under the supervision of the advisor. The thesis describes a research investigation and its results. The scope of the research topic for the thesis should be defined in such a way that a full-time student could complete the requirements for a master s degree in twelve months or three semesters following the completion of course work by regularly scheduling graduate research credits.

    Judy Waterlow, Waterlow Score, Pressure Ulcer Care and Pressure Ulcer Risk

    Pressure Ulcer Risk Assessment and Prevention

    Judy Waterlow MBE, now in her seventies, designed and researched her pressure ulcer risk assessment tool in 1985, while working as a Clinical Nurse Teacher. The tool was originally designed for use by her students.

    Judy officially retired on medical grounds in 1988, due to the increasing severity of her rheumatoid arthritis, she has continued her efforts to improve nurse education and patient care with her work for the Tissue Viability Society, serving on the Committee and organising the TVS Regional Study Days for 13 years, Challenging Arthritis, the NHS Expert Patient Programme and latterly as a Musgrove Hospital Partner. The Partners are a group of unpaid volunteers who work with hospital staff in various ways within the hospital, where their personal expertise and experience can be of value.

    This dedication to her profession both in hospital and in the community was recognised by the award of the MBE in the Queen s Birthday Honours list published on June 14th 2008.

    At a glittering Awards Ceremony hosted by the British Journal of Nursing in London on 22nd March 2013 Judy was awarded a Special Award for her services to nursing.     Hospice offers medical care toward a different goal: Maintaining or improving the quality of life for someone whose illness, disease or condition is unlikely to be cured. Each patient’s individualized care plan is updated as needed to address the physical, emotional and spiritual pain that often accompanies terminal illness. Hospice care also offers practical support for the caregiver(s) during the illness and grief support after the death. Hospice is something more that is available to the patient and the entire family when curative measures have been exhausted and life prognosis is 6 months or less.

    ​History of Hospice

    In Western society, the concept of hospice has been evolving in Europe since the 11th century. Hospice were places of hospitality for the sick, wounded or dying, as well as those for travelers and pilgrims. The modern concept of hospice, includes palliative care for the incurably ill given in such institutions as hospitals or nursing homes. but also care provided to those who would rather spend their last months and days of life in their own homes.

    It began to emerge in the 17th century, but many of the foundational principles by which modern hospice services operate were pioneered in the 1950s by Dame Cicely Saunders. when she opened St. Christopher’s Hospice in 1967. St. Christopher’s Hospice in London emphasized the multi-disciplinary approach to caring for the dying, the regular use of opioids to control physical pain and careful attention to social, spiritual and psychological suffering in patients and families.

    Within the United States. the term is largely defined by the practices of the Medicare system and other health insurance providers, which make hospice care available, either in an inpatient facility or at the patient’s home, to patients with a terminal prognosis who are medically certified at hospice onset to have less than six months to live.

    Hospice care also involves assistance for patients’ families to help them cope with what is happening and provide care and support to keep the patient at home.

    In 1969, Elisabeth wrote On Death and Dying. coining the 5 stages of death: Shock/Denial, Anger, Bargaining, Depression and Acceptance.
