To become a certified nursing assistant applicants are generally required to complete a state approved training course before they can take the state certification exams. For most of the input fields validation is done with the PHP script that will be the topic of the next section. For the email address field the validation is (also) done is another way. As you can see in the form above the type=”email” and the class form-control were used to achieve this validation. In the image below you can see the message that appears when a user inserts something other than a correct email address.

To link our contact form to the PHP script we inserted method=”post” and action=”send_form_email.php” in line 1. The file name you insert (e.g. send_form_email.php ) should be the same as the name for the PHP file that we ll create next.

PHP Script

The PHP script will pull the input from the contact form, insert this into an e-mail. This e-mail will be send to the address you insert in line 5 of the PHP script below:

The PHP script above is based on a free PHP script provided by FreeContactForm .

Optionally you can also replace the text Your email subject line in line 6 with something more appropriate. This text will be inserted into the subject field of the e-mail.

After completion of the script the user is directed to another page. Make sure that you replace the url in line 66 with the url of the page that you want to direct your users to. To make the script
pause for 2 seconds before redirecting to this url I have added a sleep command (line 65).

Save the file (make sure the name you use is the same as that in line 1 of the form e.g. send_form_email.php .)

Upload the files to a hosting server

Save both files the HTML file (with the Bootstrap modal and its contact form section) and the PHP file and upload them to a hosting server (the mail command in the script will only work on a hosting server and not on localhost).

After uploading these files you can fire the modal plugin by inserting the URL for the HTML file into the address bar of your browser.

The demo below shows what the contact modal will look like:

For obvious reasons I haven t included an e-mail address where the input from the contact form would be send to. With this demo you can see how the PHP validation works etc. After clicking on the send button in the demo you will be directed to the home page of this blog (unless the input in the email input field doesn t meet the validation criteria for that field. Then the form will pause).

In the PHP script validation rules are already included. But my recommendation is to add client-side validation to the form in the Contact Modal. With client-side validation users can first correct any mistakes while filling in the form. There is another tutorial where you can learn how to add client-side validation (and dynamic Bootstrap validation states) to the form in the Contact Modal.

Very good guide.
However the send mail function in php seems to be blocked to some email service.
Is there a way to setup authentication to avoid send mail been blocked or marked as spam?

Since I didn t immediately recognize your problem I tried to replicate it. Based on my replications the blocking / being send to the spam folder has nothing to do with the php script.

I ran 4 different trials:

In two of those trials I inserted existing email addresses in the contact form. Both of these messages arrived in my inbox.

Did you insert a valid email address in the contact form for your trials?

got everything running fine but once php runs line 66 is redirecting to http://www.xxx.yyy/send_form_email.php which is an absolutely blank and empty page. The code I have entered in line 66 is echo ; any idea why it is not bouncing back to my website?

you arrive at an empty page because you didn t insert anything between the two quotation signs after echo. The script shows an empty page because it has nowhere to go to. Use what I inserted after echo and replace the URL for my website in line 66 with that of your website and the name of the page on your website e.g. index.php).

but it still turns up an empty page. This is my first attempt at php and a php code checker indicates the following Parse error: syntax error, unexpected end of file, expecting variable (T_VARIABLE) or $< (T_DOLLAR_OPEN_CURLY_BRACES) or <$ (T_CURLY_OPEN) in your code on line 69.
Sorry I am not an expert but I am learning slowly

Thank you very much for this tutorial, I just now got my version of it to work.

I m hoping you ll spare a minute to clear something up: I wanted to plug in my own variable names, i.e. use contact_name , contact_email , and contact_msg in place of first_name , email and comments respectively. After a lot of frustration with error-checking not working and message not being sent at all, I found that I had to leave the ID and Name for the Email Address form-group (in the HTML) as email . Can you confirm that this is indeed the case: The ID and Name attributes in the Email Address form-group need to be kept named email ?

you can change the ID s to anything you want. But I would advice against changing the name s since these are related to the php-script. My guess is that your experience has to do with changing the name s and in that way breaking the links required for the php-script (which will validate and send the info from the contact form).

I ll be writing a tutorial about validation in the next week(s) in relation to another tutorial. But looking at your comment (and since the Contact Modal tut was written some while ago and can be improved) I ll probably improve this tut at the same time.

In the meantime this could provide a solution for you: you can give users feedback on errors in the form (and catch errors client-side) by adding required and a pattern to the HTML (e.g. for the First Name input tag add: pattern= /^[A-Za-z]/ required ).

With this addition to the HTML the modal will be visible (and input can be edited by users) until errors have been corrected.

When planning your semester, consider which and how many classes you want to take. The next step would be to actually register and plan your semester. To get your planning started be sure to schedule an appointment with your academic advisor, and start reviewing the following UNT resources to sign up for classes.

To stay on track for a timely graduation, we recommend you meet with your academic advisor at LEAST twice (2 times) a year:

  1. Between late September-October for Spring semester planning
  2. Between February-March for Summer and Fall semester planning

Advisors will not pick your class schedule; however you can expect to work with them to identify potential courses to fulfill your degree requirements for your major, minor and certificate options. Choice of Major

It is important to realize that requirements for medical school are not the same as requirements for a Biology major. These requirements can be met within the context of any major, so long as you plan ahead and choose appropriate electives.

Conversely, a Biology major does not in itself qualify you for medical school. It is true that a majority of our pre-medical students major in Biology or Chemistry, but in recent years we have sent students to medical school with majors in Religion, Philosophy, English, and Sociology. In every case, however, they had taken the required courses for medical school and had demonstrated the ability to do well in challenging science courses, usually through a minor in Biology and/or Chemistry.

Course Requirements for Medical School

Most medical schools require the following courses:

  • One year of Biology with lab
  • One year of General Chemistry with lab
  • One year of Organic Chemistry with lab
  • One year of Physics with lab
  • One year of English

It is always recommended that you check with each medical school to which you are applying because requirements can and do change.

We recommend that premedical students take Biology 305-306 (Human Physiology I II) during their sophomore or junior year. You should also take additional upper level science courses, in order to demonstrate that you can succeed in medical school coursework. The following courses are good choices for premedical students:

  • BIO 214 Organismal Diversity
  • BIO 260 Preceptorship in Health or Veterinary Sciences
  • BIO 311 General Microbiology
  • BIO 312 Pathenogenic Bacteriology and Immunology
  • BIO 325 Vertebrate Anatomy and Embryology
  • BIO 335 Genetics and Molecular Biology
  • BIO 337 Cell Biology
  • BIO 421 Developmental Biology
  • CHE 307 Chemistry and Cancer
  • CHE 327 Medicinal Chemistry
  • CHE 341 Biochemistry

There are also appropriate electives offered through other departments, including Kinesiology Health Sciences (KHS), Psychology, and Philosophy.

The Dreaded MCAT Exam

All medical schools also require that you take the MCAT exam (Medical College Admissions Test). The MCAT is administered several times a year. You must take the MCAT no later than the summer following your junior year in order for your application to be considered in the fall of your senior year. This means you need to be prepared for the MCAT exam in three years, not four. As minimal preparation you should complete the required courses listed above, plus the Biology 305-306 sequence, by the end of your junior year.

“Real-World” Experience

To be a viable candidate for medical school you must demonstrate that you have sufficient firsthand experience with medical practice to make an informed career decision, not just the academic ability to succeed in the medical school classroom. “Shadowing”, in which a pre-medical students observe and assists practicing doctors, is an absolutely essential part of preparing for medical school. We offer an organized shadowing experience via Biology 260 (Preceptorship in Health or Veterinary Sciences), but we also strongly recommend that you find other opportunities to obtain firsthand experience. Volunteer or paid work in a hospital, clinic, or other medical setting is an excellent way to gain experience. Other forms of community service are also very important in demonstrating your commitment to help others, even if the work is not specifically medical.

A Suggested Curriculum

The following sequence of courses will prepare you to take the MCAT and apply to medical school at the end of your junior year:

Freshman

  • Fall:
    • BIO 111- Biological Principles (with lab)
    • CHE 111- General Chemistry (with lab)
  • Spring:
    • BIO 212- Cellular Molecular Biology (with lab)
    • CHE 112, 113- General Chemistry II (with lab)

Sophomore

  • Fall:
    • CHE 201, 202- Organic Chemistry I (with lab)
    • BIO 214- Organismal Diversity (with lab), or start the physiology or physics sequence
  • Spring:
    • CHE 309, 310- Organic Chemistry II (with lab)

Junior

  • Fall:
    • PHY 111- General Physics I (with lab)
    • BIO 305- Human Physiology I (with lab)
    • Bio 260- Preceptorship in Health or Veterinary Sciences
  • Spring:
    • PHY 112- General Physics II (with lab)
    • BIO 306- Human Physiology II (with lab)
    • CHE 340 Biochemistry (with lab)

Senior

Additional upper-level courses in:

  • Biology
  • Chemistry
  • Physics
  • Psychology
  • Kinesiology
  • or other appropriate fields

You do not have to follow this schedule exactly, so long as you complete these courses by the end of junior year. However, you should plan ahead so that you are taking two laboratory courses per semester. For example, if you start the Chemistry sequence your sophomore year you might also take Physics sophomore year otherwise you will be taking Organic Chemistry, Physics, and Physiology, all with labs, concurrently in your junior year. Of course, you will also have to take additional courses to meet the requirements for your major. Again, plan ahead so you don t have too many lab courses in any one semester.

Many students choose to wait one or more years between college and medical school to pursue other interests or to further prepare for medical school. If you want to do so you can spread the courses listed above over four years. Be aware, however, that if you take the MCAT in your senior year you will not be able to start medical school until one year after you graduate.

Pre-Med Curriculum with a Biology Major

For a Biology major you will need to: (1) complete the Biology core, (2) take the Biology junior and senior seminar courses, and (3) take at least 15 hours of 300- or 400-level Biology courses while still meeting the requirements described above. The following plan of study will allow you to meet these requirements and be prepared for the MCAT exam at the end of your junior year:

Freshman

Practical Nursing

The Practical Nursing (PN) Program prepares the individual to assist in providing general nursing care which requires the basic knowledge of the biological, physical, behavioral, psychological, sociological sciences. This care is performed under the direction of a registered nurse, licensed physician or dentist.

Students desiring to enter the PN program will be selected based on the following criteria:

  1. Applicants must be at least 18 years of age.
  2. Applicants must have a high school diploma or GED equivalency.
  3. Applicants must have an enhanced ACT composite score of 16 or greater.
  4. Applicants must have a Northwest application on file with the Registrar’s Office.
  5. Once a prospective student has a PN application on file with the PN Program, an opportunity to sit for the Test of Essential Academic Skills (TEAS) can be scheduled by calling the Career-Technical Division for each respective campus.
  6. Applicants must complete and obtain the required score for the TEAS test.
  7. Once accepted into the Program, Applicants must complete the following:
    1. Student Health Record,
    2. Negative 5-panel drug screen,
    3. TB skin test,
    4. Copy of immunization records,
    5. Completed Hepatitis B vaccine waiver form,
    6. Provide certificate of Liability Insurance for a health provider,
    7. Obtain CPR health provider card,
    8. Complete health care criminal history background affidavit.

The PN Program accepts applications from mid-January through April 30 of each year. Testing begins each February.

Program Withdrawal
If a student finds it necessary to withdraw from the program for any reason, the official steps as outlined in the NWCC Bulletin must be followed. A Student who properly and officially withdraws will receive a grade of “W” (official withdrawal) in all classes. Withdrawal must be made prior to exam week. An unofficial withdrawal is attributed to a student who leaves the program without going through the published withdrawal procedures. This unofficial withdrawal will result in the student receiving “F” grades in all classes.

Readmission Policy
Any student who is dismissed from the Practical Nursing program (regardless of how far along they are in the program) will not be given any credit for work that has been completed before the dismissal. Upon re-admittance, the student will start over completely as if they are beginning the program for the first time. A grade of 80% or above is required in all courses in order to progress. The student who does not achieve these criteria will be allowed one chance to reenter the Practical Nursing Program.

Students who complete the program requirements, as identified by the Mississippi Department of Education, will be eligible to apply to the Mississippi Board of Nursing for licensure to practice as a licensed practical nurse. All applicants should be advised that according to the Mississippi Nursing Practice Laws and Rules and Regulations an individual’s application to write the licensing exam may be refused if the applicant has been convicted of a criminal offense under any provision of Title 97 of the Mississippi Code of 1972.

Course 5: Medical Billing Insurance Claims Process

The medical billing insurance claims process starts when a healthcare provider treats a patient and sends a bill of services provided to a designated payer, which is usually a health insurance company. The payer then evaluates the claim based on a number of factors, determining which, if any, services it will reimburse.

Let’s briefly review the steps of the medical billing procedure leading up to the transmission of an insurance claim. When a patient receives services from a licensed provider, these services are recorded and assigned appropriate codes by the medical coder. ICD codes are used for diagnoses, while CPT codes are used for various treatments. The summary of services, communicated through these code sets, make up the bill. Patient demographic data and insurance information are added to the bill, and the claim is ready to be processed.

Processing Claims

A number of technical protocols and industry standards must be met for insurance claims to be delivered expediently and accurately between medical practice and payer.

Medical billing specialists typically use software to record patient data, prepare claims, and submit them to the appropriate party, but there isn’t a universal software application that all healthcare providers and insurance companies use. Even so, insurance claims software use a set of standards, mandated as by the HIPAA Transactions and Code Set Rule (TCS). Adopted in 2003, the TCS is defined by the Accredited Standards Committee (ACS X12), which is a body tasked with standardizing electronic information exchanges in the healthcare industry.

There are two different methods used to deliver insurance claims to the payer: manually (on paper) and electronically. The majority of healthcare providers and insurance companies prefer electronic claim systems. They are faster, more accurate, and are cheaper to process (electronic systems save around $3 per claim). But because paper claims have not yet been completely removed from the insurance claims process, it is important for the medical biller and coder to be well versed with both electronic and hardcopy claims.

Filing Electronic Claims

Certain technologies have been introduced into the system in order to expedite claim processing and increase accuracy.

Some healthcare providers use software to electronically enter information into CMS-1500 and UB-04 documents. Using “fill and print” software eliminates the possibility for unreadable information. This software may also include certain types of “scrubbing,” or tools that check for errors in the documents. While these tools do decrease the amount of errors made in filling out claim forms, they are not always 100 percent accurate, so medical billers should remain diligent when filling out forms using software.

Optical Character Recognition (OCR)

OCR equipment scans official documents, electronically isolating and recording information provided in the different fields, and transferring (or auto-filling) that information into other documents when necessary. While OCR technology helps make hardcopy claim processing much more efficient, human oversight is still needed to ensure accuracy. For instance, if the OCR miscalculates a simple digit in a medical code, that error must be flagged and manually corrected by a medical billing specialist.

Note that when OCR equipment is not available, it is possible for a medical billing specialist to manually convert CMS-1500 and UB-04 documents into digital form using conversion tools called “crosswalks” (note that the same term applies for tools used to convert ICD-9-CM codes to ICD-10-CM). You can find crosswalk references from a number of different sources.

Filing Manual Claims

Paper claims must be printed out, completed by hand, and physically mailed to payers. The healthcare industry uses two forms to submit claims manually. Since processing paper claims requires more manual interaction with forms and data, the opportunity for human error increases compared to electronic claims. Documents can be printed improperly, and handwritten codes can be incorrect or illegible. The forms can also be mailed to the wrong address, with insufficient postage, or disrupted by logistical complications with the delivery services. These errors are costly for the healthcare provider, often resulting in form resubmission (a time-consuming process) and payment delays.

Generally, healthcare professionals like family physicians use form CMS-1500, while hospitals and other “facility” providers use the UB-04 form.

The CMS-1500 is the universal claim form used by non-institutional healthcare providers (private practices, etc.) to bill Medicare for Part B covered services and some Medicaid-covered services, and is accepted by most health insurance providers. The CMS-1500 is maintained by the National Uniform Claim Committee (NUCC), and was previously updated to include National Provider Identifiers (NPIs), or unique numbers required by the Health Insurance Portability and Accountability Act (HIPAA).

Form CMS-1500 contains all the basic information needed to submit an accurate claim. This includes fields for the patient’s demographic information, insurance information, and boxes in which to provide medical codes and corresponding dates of service. Certain boxes are used exclusively for Medicare and/or Medicaid. It is important to note that different payers may provide different instructions on how to complete certain item numbers. The medical biller and coder should be familiar with specific payer requirements before filling out the form.

Form UB-04, also maintained by the NUCC, is very similar to the CMS-1500, but it is used by institutional healthcare providers, such as hospitals. Like the CMS-1500, the UB-04 is used in lieu of electronic claims when the facility meets any number of exceptions granted by the ASCA. It is the responsibility of the facility to self-assess whether these designated exceptions apply to their operation, granting usage of manual claims. Also similar to the CMS-1500, certain payers may not require all fields, or data elements, to be completed.

The role of clearinghouses

Once a file is created using these standards, it is usually sent off to a clearinghouse. The clearinghouse is a third-party operation that primarily acts as a middleman between healthcare providers and insurance carriers.

Think of the clearinghouse as a central hub, or a single location where all claims are sent to be sorted and directed onward to all the various insurance carriers. Typically, clearinghouses use internal software to receive claims from healthcare providers, scrub them for errors, format them correctly in accordance with HIPAA and insurance standards, and send them to the appropriate parties. Clearinghouses generally keep medical practices in the loop during this process by providing reports on the status of claims.

This third party is necessary because healthcare providers typically have to send high quantities of insurance claims each day to a variety of different insurance providers. Each of these insurance providers may have their own submission standards. If a medical practice’s billing staff was solely responsible for transmitting insurance claims under both insurance and HIPAA requirements, the potential for error would increase dramatically, not to mention the time required for formatting each claim to specific insurance carrier.

When choosing a clearinghouse, a healthcare provider should consider two main factors:

  • Does the clearinghouse have the capability to work with the insurance providers the practice works with most often?
  • Can the clearinghouse accommodate claims transmissions from the insurance provider’s practice management software?

Confirming these questions ensures that all transmissions run smoothly.

Wrapping Up Course 5

Healthcare providers prepare insurance claims using information provided in the patient’s bill. Occasionally, the claim is prepared manually and sent by mail. In most cases, the claim is sent electronically (having either been prepared using claim software or scanned from a hard copy) to a clearinghouse. The clearinghouse checks the claim for errors, formats it according to HIPAA and insurance guidelines, then transmits it to the appropriate payer, while also sending a report back to the healthcare provider.

After the claim has been evaluated, the insurer must provide both the patient and healthcare provider with an Explanation of Benefits (EOB). The EOB breaks down the adjudication process, showing the dates of service, procedures and charges, patient financial responsibility, and the amount paid to the healthcare provider. At this point, the health insurer sends payment to the healthcare provider, usually in the form of an electronic fund transfer.

The insurance claims process can be complex. Fortunately, there are tools to help complete insurance claims on a day-to-day basis. Medical billing professionals who are familiar with these tools and all documents (both paper and electronic), industry standards, individual insurance company regulations, clearinghouse procedures, and the adjudication process will be prepared to succeed.

Message: You can start your search here: CNA programs in Wayne County, MI. You have to contact the schools and ask if they also administer the CNA test although you may find that this is actually rare. I think that once you contact them they will provide all the answers you need. In fact I don’t think there are many schools which require you to take more than the minimum required number of training hours for your state. So please don’t hesitate to call them and ask about the exam, certification, etc. After all you are the one paying and they will most likely want to have you as a student.

We are so lucky to have an abundance of fresh delicious fruit and vegetables in Australia. It is easy to forget with so many imported foods what fruits and vegetables are actually in season at times.

We have been investing bucket loads of time energy into researching and creating some yummy dishes with body and earth in mind. We hope to share some of these dishes with you in one of my classes. (Oh what a hardship!)

Corporate team building classes and workshops can take up to 30 people and can be held in Seaforth or at your chosen venue.

Workshops and retreat numbers may vary from the above and will be clearly stipulated in the description.

Louise is a nutritionist, speaker, caterer and culinary educator who enjoys a vast array of cuisines. Some classes are more instructional than others and have a practical or nutritional focus to them. If you’re wanting a class that is a little less educational and more fun focused, there are also many exotic cuisines to choose from.

    Online Colleges in Kentucky

    Online higher education has been on the rise in the state of Kentucky in recent years thanks to efforts by the Council on Postsecondary Education. The council has an entire committee called the Distance Learning Advisory Committee, dedicated to promoting online schools in Kentucky. The committee works to improve and implement the necessary infrastructure to permit students to take courses and earn their degrees remotely. The committee also discusses ways that they can improve existing online schools in Kentucky.

    Higher Learning Initiatives

    GEAR UP Kentucky

    GEAR UP Kentucky is an education initiative aimed at increasing the number of people who go on to state colleges after graduating from high school. The Council on Postsecondary Education was given $26.9 million by the Kentucky state government to spend over a six-year period to cultivate a culture among younger students which encourages them to earn a higher education. Working with school educators and administrators, the GEAR UP Kentucky initiative strives to improve college enrollment by addressing five major areas in primary and secondary education. These include, giving students the aspiration to attend college; providing the educational rigor to prepare them; setting up expectations for higher education options; holding schools accountable for the success of students; and sustaining the effort to create a college-focused student body.

    Regional Stewardship Program

    The Regional Stewardship Program is a Kentucky-based initiative whereby the state universities support, build up, and reach out to the communities around them. Faculty, staff, and students work through the Regional Stewardship Program to lend their resources to surrounding communities in need. The initiative promotes development throughout communities in the state with the belief that improving the well-being of residents will result in a more robust and talented student body. Funds are available to state universities that allow them to organize, execute, and promote substantial community outreach and support initiatives.

    P-16 Councils

    P-16 Councils are coalitions of citizens, business owners, educators, and local officials who try to craft initiatives aimed at improving education at all grade levels, college included. The various P-16 Councils throughout Kentucky share the mutual goal of making education better and more attainable for all citizens. Among the many initiatives endorsed by P-16 Councils is Project Lead the Way. a national organization that works with educators and state legislators to promote science and math education among younger students, paving the way for tomorrow s engineers, scientists, and mathematicians.
