Jefferson Health forms as merger closes, sets aggressive innovation plans

Thomas Jefferson University Hospitals and Abington Health System have completed their merger, forming Jefferson Health to compete with Philadelphia’s University of Pennsylvania Health System, the largest in the city. The question now is whether the system will be as much of an industry disruptor as CEO Stephen Klasko, MD, has promised.

“Jefferson Health is truly different, because now patients can choose an organization that marries the nationally recognized and renowned academic medical center of Thomas Jefferson University with the outstanding clinical reputation and community connectedness of Abington,” said Klasko.

Jefferson Health, with a medical school and five hospitals, is pursuing a mission of education, research and advanced medicine. It’s on a slightly smaller scale than competitor Penn Medicine, which is researching diseases and treatments with $409 million in NIH funding, and offering access to clinical trials and new treatments through a soon-to-be five-hospital system with the planned acquisition of Lancaster General Health on the horizon.

Jefferson Health has been designed to extend the expertise of Jefferson clinicians to suburbanites through telehealth, urgent care centers and Abington’s three community hospitals and clinics, rather than set up a regional referral network for an academic medical center to keep doing its best work at an urban hospital complex, Klasko said.

“This new urban-suburban hub-and-hub model we believe is the first in the country,” Kasko said. “If you look at a lot of mergers, it’s a hub and spoke, an academic medical center in the city, and people have to travel and it increases the costs.”

Jefferson Health’s goal, Klasko said at a press conference, is “going from a Blockbuster model to Netflix model, bringing Jefferson care and Jefferson and Abington care to the patients as close as they can be.”

The new system, spanning Philadelphia and its northern suburbs, features five hospitals (including Abington Memorial on the edge of the city and Jefferson’s downtown 950-bed University Hospital), nine outpatient centers and four urgent care centers, all staffed by 19,000 employees and 3,370 physicians. Among the executives at the new health system are Praveen Chopra, chief information and transformative innovative environment officer; Anne Boland Docimo, MD, chief medical officer; and John Ekarius, chief strategy officer. The board includes 11 trustees from the old Jefferson, 11 from the old Abington, plus two independent members.

Klasko said Jefferson and Abington did not really have to combine to survive. Among the dozen-plus health systems and independent hospitals in metropolitan Philly, both were in solid financials prior to the merger. Jefferson had a net income of $103 million on $2.1 billion in revenue in fiscal year 2014, while Abington brought in $17.6 million on $774 million in revenue.

But last summer, when Klasko dined with Larry Merlis, Abington’s CEO and COO of the new system, there was a connection that suggested the two organizations shared a vision for the future. “After the glass of wine, it became more obvious that Jefferson and Abington would be a great fit,” Klasko said.

The Netflix model

A 61-year-old OB-GYN, MBA and Philly native, Klasko has done quite a bit in stoking Jefferson’s brand in his two years on the job as CEO. He lambasts the arcane, dysfunction aspects of American healthcare — from the six-figure debt for patients to phone call scheduling systems,— and points to consumer technology as inspiration. “Why can I be in my pajamas the day after Thanksgiving watching Game of Thrones and doing all my holiday shopping, but if I have a stomach ache, I still have to get on the phone and hopefully somebody will see me two days from now?” Klasko said in an interview earlier this year.

“I see this as an absolutely seminal moment in healthcare,” he said. “We’re going to change the DNA of healthcare one physician at a time,” he also said in a TEDx Talk, outlining a vision for “Jefferson 3.0.”

Klasko has been saying that American healthcare needs to change for the better part of a decade, since he was head of the University of South Florida College of Medicine. His record there was mixed. One former colleague told Philadelphia Magazine that Klasko’s “disruptive innovation” agenda was seen as “just disruptive.”

At the Villages retirement community, Klasko spearheaded a $4 million USF medical clinic with the goal making it “America’s Healthiest Hometown.” USF pulled out the project last June, taking a $5 million loss. Another initiative, the $38 million Center for Advanced Medical Learning and Simulation, lost $2 million for the 2013-14 fiscal year, but is still seen as part of needed changes in the ways medical students learn to become doctors, nurses and caregivers.

Competing in Philly

At Jefferson, Klasko said he has a mandate to evolve the enterprise, which “means everything from literally changing how we select and educate students” to “changing the most expensive place to can get care, the urban academic medical center,” he said. “We believe that 65 percent or so of patients who end up in a hospital’s emergency rooms don’t need to be there, and not just because it could not just be five hours of a patient’s life but $1,500 of their deductible.”

The hub-and-hub health system can be the way to “get patients to the most efficient and effective place for them to get care,” Klasko said. “That might be their home with telehealth. That might a Jeff Connect urgent care center. That might be a freestanding ER, or if they’re really, really sick, they should go to the most expensive, high acuity ER at the hospital.”

Whether Klasko’s ideas and Jefferson’s vision translate into more affordable healthcare remains to be seen, said Robert Field, a Drexel University health researcher who writes the Field Clinic column in the Philly Inquirer. For one thing, Field said, neither Jefferson nor any other regional provider system has made headway in improving patient billing, although the region’s largest insurer, Independence Blue Cross, is trying to make progress on the price shopping front.

Jefferson is also not the only area health system looking to create an integrated health network spanning the suburbs and center city Philadelphia. Jefferson bought the naming rights to a downtown train station, but all across the region, residents are beckoned with advertising for systems such as Temple Health, Einstein Healthcare Network, Main Line Health, Doylestown Health, Jefferson and Penn Medicine. The competition between Penn Medicine and Jefferson as the largest and second largest academic medical centers in the region has been explicit, though still friendly, said Field, who worked in management at the Penn’s health system in the late 1990s.

Jefferson and Abington have a slight advantage on the retail clinic approach. Like other metro areas, Philly has dozens of urgent-care clinics, though only a few are operated by major hospital systems.

That, along with the expanded telehealth options Jefferson is rolling out with the American Well on-demand telemedicine company, could put Jefferson ahead in an area that Klasko thinks is growing more quickly than some healthcare executives might like to acknowledge.

In 2010, Klasko and a group of academic medical center leaders were at a conference when Walgreens announced the launch of its walk-in clinics. Klasko remembered many of the deans laughing it off: “What a stupid business model. Who’s going to go to a drugstore to have their kid be seen with an earache?”

Billions of dollars later, Klasko said, retail primary and urgent care clinics are one of the fastest growing parts of healthcare, and some providers complain that they are taking away high-margin, low-acuity services.

“The reason isn’t because everybody was excited about going to the drugstore to have their kid be seen. The reason was, back then, if your kid had an earache you would be told by your pediatrician in many places that we could see you in two days,” Klasko said. “Well by then, your kid was either better or had gone to the emergency room.”

Which one you choose depends on what you want to do next. The Associate of Arts degree will include more liberal arts classes. An Associate of Science degree will include more science courses. An Associate of Applied Science degree focuses on the technical aspect of the work and is designed for people who want to acquire a specific skill or vocation and go right to work. It is considered a “terminal” degree, meaning not many credits will carry over toward a bachelor’s degree. So, if you want to continue your education at some point, it is best to consider an Associate of Science or an Associate of Arts degree. In all three cases you can choose to get a general health administration degree or you can choose a concentration in a specific area, such as pharmacy practice or medical records.

Course Requirements for a Health Administration Associate Degree

The specific courses you will take will depend on what degree you are pursuing and the area in which you plan to concentrate. Most programs require certain core courses in the degree area, such as classes on how the health care system works, technology for managing health care practices and functions, basic medical law and ethics, mathematics, medical terminology and introduction to anatomy. If you are on the arts track, you might take some courses in the humanities, English and writing, civilization, history, sociology or a foreign language. Those on a science track might enroll in introduction to biology, chemistry, psychology and even physics classes. If you are planning to concentrate in the general business aspects of the field, you may take additional courses in business and administration, organizational theory, customer service and marketing. If you seek an area of concentration, you will want to add electives in a specific field such as pharmacology or information technology. Typical degrees require about 60 credit hours of work.

Job Prospects with a Health Administration Associate Degree

Most students look for a job when they graduate, even if they plan to go on and get a bachelor’s degree. Work experience can help you get into a better school if you are continuing your education. If you plan to stay in the workforce, experience is the key to better-paying jobs. Getting an entry-level administrative job in a doctor’s office or in a hospital is a common choice, but it isn’t your only option. Insurance companies, clinics, pharmacies and long-term care facilities also look for people with health care administration credentials. You may find yourself doing a little bit of everything in a small physician’s practice. In a larger facility, you may be more focused in an area such as managing patient records, coding for insurance billing or managing inventory at a pharmacy.

    Effective Medication Use
    To assure the meaningful use of standardized electronic health records (EHR) that supports safe, efficient, and effective medication use, continuity of care, and provide access to the patient-care services of pharmacists with other members of the interdisciplinary patient care team.

    Pharmacist’s Role in HIT
    To assure the pharmacist’s role of providing patient-care services are integrated into the National HIT interoperable framework.

    The Collaborative’s Guiding Principles

    • Identify (through the consensus work of expert panelists) the minimum data set and functional electronic health record (EHR) requirements for the delivery, documentation, and billing of pharmacist-provided medication management services. Such requirements include access to key medical information such as laboratory data, and bidirectional communication flow among all practitioners;
    • Structure and support implementation of a Pharmacy Practitioner HIT roadmap (Roadmap). The Roadmap is a document that directs and establishes benchmarks. These benchmarks will describe the development, implementation and application of technology in an efficient and effective manner for pharmacists to affect improved medication use;
    • Build cooperative relationships within pharmacy and among pharmacy and other stakeholders to communicate and advocate for the Pharmacy Practitioner minimum data set and Roadmap leading to a certified EHR as defined in the Federal Register [1] .
    • Ensure pharmacy representation on key HIT-related committees and workgroups.

Employment as a Health Care Administrator is expected to grow faster than average with the fastest growth in home health agencies, residential care facilities and practitioners’ offices and clinics.

    Our college healthcare administration degree program has been designed in two formats, one for Michigan college students who enter the healthcare administration degree program either as college freshmen or students that transfer into the health care administration degree program after having completed an Associate degree and the second for health care; students who wish to complete a bachelors degree in health care administration while enrolled in a health care Associate degree program.

    Advance Your Career with a Healthcare Administration Degree

    Students that major in healthcare administration are required to take college courses in health care management, marketing, accounting, quality assurance, health care finance, health care planning and long-term care management that prepares them to work in many areas of the health care industry. Graduates of the Healthcare administration degree program, are able to enter graduate school to earn a masters degree in health care administration.

    Our college Health Care Administration program also is offered in Grand Rapids, Michigan. In the Grand Rapids Michigan health care administration degree program, the college health care administration courses are offered in the evenings. General college education requirements may be taken at a local Michigan college or university and transferred to fulfill the Ferris State health care degree requirements. A minimum of 30 semester credit hours must be earned at Ferris State in order for the college health care systems administration degree to be granted.

    Health Care Administration College Graduates Get Great Jobs

    The term “health services manager” encompasses health care professionals in many different positions who plan, organize, coordinate, and supervise the delivery of health care. Health services managers include both generalists (health care administrators who manage or help to manage an entire facility or system), and health specialists (health care managers in charge of specific departments or services found only in the health care industry).

    Earnings of health services managers vary by type and size of facility, level of responsibility and geographic region. The median salary for college graduates with a bachelors degree in healthcare administration in small group practices with fewer than seven physicians is about $55,000 and, for those in larger group practices with more than seven physicians, about $77,000. Heads of clinical departments salaries average between $54,500 for respiratory care to $70,400 for rehabilitation services. Nursing home administrators have an annual median salary of $49,500.

    Applicants for admission to the Healthcare Administration degree program must have a 2.0 GPA in high school or college course work and one year of high school algebra with a ‘C-‘ or better. Students that major in health care administration can enter the health care administration bachelor degree program from any associate degree program or directly from high school.

    The Health Care Systems Administration degree program at Ferris State leads to a bachelor of science degree. Graduation from the health care administration major, requires a minimum 2.0 GPA overall. Also, college students that major in health care administration must either have an ACT math sub score of 24 or better, complete a basic math class or pass a proficiency exam.

    The Healthcare MBA: One Fast Track in a Slow Economy

    The healthcare track for MBAs has never been a hot one until now. Believe it or not, healthcare companies can’t find enough MBAs to fill open positions in this $2.26 trillion industry. says Marjorie Baldwin, head of Arizona State University’s School of Health Management and Policy. Hospital administration and healthcare practice management may still lack the glamour of top consulting gigs, but the opportunities now go beyond just hospitals B-school grads can find roles in everything from biomedical startups and e-health ventures to consulting and pharmaceutical R D.

    Even as the country overall lost another 598,000 jobs in January, according to the Bureau of Labor Statistics, healthcare was one of just three industries to expand its workforce. In fact, healthcare services added 372,000 jobs throughout 2008, and a good chunk of those were on the business side. At Kaiser Permanente, for example, a third of the 16,000 to 18,000 positions that open up each year are in management and operations.

    Top feeder schools:

    The Wharton School at the University of Pennsylvania: Wharton s long-established Healthcare Management program excels at sending grads to lucrative industries beyond the usual hospital administration jobs 29 percent of the program s 2008 grads ended up in biotech and pharmaceuticals.

    Duke University s Fuqua School of Business: The Health Sector Management program enjoys close ties to North Carolina s Research Triangle, one of the fastest-growing clusters of biotech firms in the country.

    Northwestern University s Kellogg School of Management: Kellogg s 300-member student healthcare and biotech club hosts the annual Business of Healthcare conference, a premier networking and recruiting event in the health services industry.

    Major MBA recruiters:

    IMS Health: The 8,000-employee consulting firm works with nearly every major pharmaceutical and biotech company in the world. And according to Clint Wolfe, global director of talent acquisition, the company is as eager to take on new MBAs as it was before the recession.

    Amedisys: The nursing management company has projected 2009 earnings above analysts expectations and forecasts continued growth this year due to a series of strategic acquisitions. Amedisys is thriving in part because during recessions patients opt for care at home as opposed to pricey hospital visits.

    Amgen: Amgen Inc. the world s largest biotechnology company, targets new grads by hosting an MBA Leadership Program as well as summer internships where students can expand their studies in biotech sales, marketing, and finance.

    HCA: Armed with 180,000 employees, the Hospital Corporation of America is the largest private operator of healthcare facilities in the world. Eager to snap up recent grads, the company offers special executive-development programs focusing on everything from finance to hospital operations/administration.

    The job outlook for new MBAs

    Healthcare organizations typically look for clinical experience before business acumen, says Baldwin. This year, though, business acumen is a lot more front and center, with healthcare firms desperate to manage rising costs. They are just now realizing how much they need business people running [their] organizations as opposed to clinicians, Baldwin says. That leaves the industry facing a huge hiring opportunity recruiters and university administrators report that a much larger pool of MBAs are pursuing careers in healthcare services, biotech firms, and pharmaceutical companies.

    With more B-school grads interested in the field, the competition has pushed the bar higher for potential hires. In addition to requisite skill sets in finance, marketing, business development, and management, MBAs still need some clinical, scientific, or healthcare systems knowledge to stand out. Top schools like Wharton and Kellogg prep students with courses like biomedical marketing, e-health business models, and healthcare economics and finance, which because of the multipayer system is strikingly different from finance in other fields. This experience allows graduates to step into product development, product marketing, supply-chain analysis, and senior management roles. It doesn t matter whether it s a Johnson Johnson, a Pfizer, or other firm, they re always looking to hire someone who can analyze the science or understand a patent portfolio, says Dr. Gail Naughton, dean of San Diego State University s College of Business Administration. You can add value from day one.

    In a down market, companies are also looking for candidates who are willing and able to perform multiple jobs. As we and other healthcare firms try to manage costs, we need a workforce that can change roles, says Dana Pulliam Gharda, senior manager of university relations for the California-based biotech firm Life Technologies. Though the company will hire 40 to 50 percent fewer MBA grads this year, Gharda will need product management candidates who can coordinate the marketing and business development of new products on the fly as market demands shift.

    Data Centers and HIPAA Compliance

    There have been questions about what role a data center plays when it comes to HIPAA. We want to address what requirements and obligations data centers have when working with clients in the healthcare industry.

    First of all, what is HIPAA? The acronym stands for the Health Insurance Portability and Accountability Act of 1996, enacted to protect the health information of patients. When you visit a doctor’s office or the emergency room at your local hospital, all the people seeing your medical history have signed some sheet of paper, promising to keep your information private. This means to disclose healthcare information, they must have your permission (or authorization from the proper authorities in cases of child abuse, etc.). HIPAA also covers how physical and electronic data is handled and secured. Healthcare entities must backup their data and have a disaster recovery plan in place. This is where data centers come in.

    The Health Information and Technology for Economic and Clinical Health (HITECH) Act was enacted on February 17, 2009. This Act requires covered entities to disclose breaches in Protected Health Information (PHI). The covered entities and their business associates that “access, maintain, retain, modify, record, store, destroy, or otherwise hold, use, or disclose unsecured PHI” are required to notify the Department of Health and Human Services or any breaches. The business associates must notify the covered entity of a breach who in turn notifies the individuals involved (patients) and the HHS if more than 500 individuals were affected. From the statement above, data centers like Data Cave, would be considered a business associate.

    The problem is there is much to speculate on what this actually means. Some data centers use HIPAA compliance as a marketing tool. Let me make something clear, there is no certification for HIPAA. A data center can be HIPAA compliant, which is what we at Data Cave consider ourselves. Some pay an outside source to come in, look around, and put their stamp of approval on the facility. For Data Cave, meeting HIPAA compliance means limiting people with access to equipment, including our own staff. This also means notifying the proper channels when someone has been near a healthcare entity’s equipment. With most healthcare companies, they are going to want to manage their own equipment, which means our staff wouldn t need to touch it anyway. However, for a data center doing managed services, facility staff would be responsible. In that case the facility would enter into an agreement with the customer to maintain confidentiality. In the event of a breach, whether virtual or physical, a data center would notify the customer (the covered entity) who would, in turn, notify the HHS if applicable.

    In other words, no one can claim HIPAA certification. To take it a step further, the essence of a data center is to be secure; so in that case, aren’t we all HIPAA compliant?

    While Hollywood rolls out the red carpet for the Oscars tonight, the president and first lady hosted a night of glamour of their own. But instead of movie stars, the guests at the White House tonight were the nation’s governors.

    It’s an annual tradition for the president to invite the nation’s governors to the White House for a dinner. Tonight’s Governor’s Ball was a first for President Trump and first lady Melania Trump as host and hostess at the White House for a gala event.

    “I hear this is a record number of governors, 46, and that’s the highest that have ever showed up for this evening,” the commander in chief said during a toast.

    The theme of the dinner was Spring’s Renewal, with the first lady saying in a statement, “The scents of jasmine and roses fill the air as we give thanks for this great nation and the glory of renewal.”

    She also said the night will be an opportunity to leave political labels behind and unite.

    “I am proud to invite all the governors to the White House for this important annual event,” Melania Trump said. “Tonight, we come together as one nation, leaving political labels and partisan interests behind.”

    However, Donald Trump briefly touted the accomplishments of his young presidency, highlighting border security, saying, “I can say that after four weeks, it’s been a lot of fun accomplished, but we’ve accomplished almost everything we’ve started out to accomplish. The borders are stricter, tighter.”

    He teased the group’s upcoming discussion of former President Obama’s signature legislative achievement, the Affordable Care Act.

    “Tomorrow we’re going to meet, and we’re going discuss things like perhaps health care will come up, perhaps, and I think we’ve made a lot of progress on that, and we’re going to have a speech Tuesday night, and we’re going to be speaking very specifically about a very complicated subject,” Trump said. “We’re going to have it fixed, and we’re going to repeal and replace, and I think you’re going to see something very special.”
