The Problem of Circumcision in America

Many circumcised men are unaware that they have been harmed by circumcision because they do not understand how the intact (non-circumcised) penis functions and/or they do not know how to identify their circumcision damage. Many circumcised men fail to recognize their own circumcision damage and assume that peculiar things about their penis are ‘normal’ or ‘a birth defect.’

This tightness and the unnatural dryness of the penile shaft and head causes some men (and their partners) pain, abrasion, and bleeding during sex. Breaks in the skin are an effective way to transmit and contract sexually transmitted diseases, including HIV.

The glans (penile head) is normally an internal organ protected by the moist mucosal tissue of the prepuce (foreskin). Without the foreskin, the glans is exposed to the outer environment (air, soap, clothing, sun, etc.). The glans dries out and develops several extra layers of skin (keratinization). 1 Compare the dry, cracked appearance of the glans of a circumcised penis (upper right) with the moist mucosal end of the glans of an intact penis (lower right). The unnatural dryness of the circumcised penis can cause abrasion and bleeding during sex, whereas the moistness of the intact penis makes sex more comfortable, especially during penetration. For women with circumcised male partners, the problem of ‘vaginal dryness’ during sex may also be partly attributable to the unnatural dryness of the circumcised penis.

Besides removing the densely nerve-laden foreskin, circumcision removes 50% of the penile shaft skin and associated nerve endings. 2 The exposed glans then keratinizes, causing further loss of sensation. Many circumcised men in the Awakenings survey 3 reported that desensitization caused them to abandon or bypass the subtler pleasures of genital foreplay in favor of immediate intercourse, which would offer them greater stimulation. They often hurried through intercourse, however often needing extraordinary and sometimes violent thrusting to obtain sufficient stimulation for both pleasure and orgasm. Other men reported frequent reliance on behaviors offering more stimulation than vaginal sex (e.g. oral sex, anal sex or masturbation) or compensating for diminished quality of sexual response with quantity (sexual compulsivity).

Eventually, keratinization occurs even after adult circumcision. It can also occur in intact men with short foreskins or those who habitually wear their foreskin in the retracted position.

Imagine how different female sexual response would be if the clitoral hood (female foreskin) and the labia were removed. Exposure of the clitoris to the constant effects of the outer environment would approximate the effects of male circumcision.

    The New York Medical Malpractice Law Blog #medical #malpractice #lawyers #ny

    New York State is far from the worst when it comes to how victims of medical malpractice are treated in court. For example, we do not have set limits on the amount of money damages that can be awarded to a plaintiff for so-called non-economic damages, that is, the damages that can be awarded to compensate for the victim s pain and suffering. Other states do, in the form of caps, often placed at $250,000.

    But medical malpractice victims can be hurt, a second time, by unrealistic limitations in the time they have to bring an action in court. Barring exceptional circumstances, malpractice victims who miss such a deadline, called a statute of limitations, are forever barred from having their complaint heard in court. In New York, a victim of medical malpractice has 2 and 1/2 years from the date of the malpractice to start a lawsuit. That may sound like a sufficient amount of time to realize that a medical error was made, and to hire a lawyer to do something about it.

    But what if, through no fault of your own, you don t discover the malpractice until more than 2 and 1/2 years have passed? Right now, you, and every other New Yorker, are powerless to seek compensation. But there is a groundswell of support for a law that may change that unfortunate situation. It is called Lavern s Law, after a New Yorker who fell victim to the unfairness of the current statutes of limitations. This story in the NY Daily News provides some background. Lavern Wilkinson had a curable case of lung cancer, but her doctors failed to diagnose it on time. By the time they, and Lavern, realized their mistake, it was more than 2 and 1/2 years from the time of the misdiagnosis, and neither she, nor the family she left behind, had any recourse in our court system.

    As this article in The Journal News points out, New York is one of only 6 states in the U.S. that fails to use a date of discovery rule for medical malpractice victims, i.e. a rule that allows a malpractice victim to bring suit within a set time from the date of discovery of the malpractice and not just the time of the malpractice.

    As my colleague, Eric Turkewitz, stresses in his post on the topic, it s time to pass Lavern s Law, before this nonsensical void in our law works to deprive someone else of his or her right to hold a negligent doctor accountable. Eric s post provides helpful information on how you can help advocate for getting Larvern s Law to a vote on the Senate floor, and I hope you will do so. New York s victims of medical malpractice deserve better.

    It is by now old news that President Trump s top lawman, Jeff Sessions, ordered most of the country s U.S. Attorneys to clear out, immediately or sooner, a couple of days ago. This normally calls for the submitting of resignations. The U.S. Attorney for the Southern District of New York, Preet Bharara, did not go peacefully however, and forced Sessions to fire him instead, for reasons that will likely come into focus in the coming weeks. One reason for the highly charged departure, though, is quite public. President Trump had met with Mr. Bharara, and had personally requested that he stay on to work during the Trump Administration s tenure. Therefore, the change of position apparently took the U.S. Attorney by unpleasant surprise.

    Not everyone in Mr. Bharara s position was treated that way. In fact, the U.S. Attorney for the Northern District of New York was told that he could stay on. in order to accumulate enough time for his pension to become effective, and to ensure an orderly transition. That was a generous and reasonable thing to do: generous concerning Mr. Hartunian s pension; and reasonable because leaders of federal prosectors offices need time to effectuate an orderly transition, and insure that investigations in progress continue to move forward under proper guidance.

    The manner in which Mr. Sessions carried out his removal of most sitting U.S. Attorneys was abrupt, making the possibility of achieving orderly transitions at each office slim. From here, I can t imagine why he chose to do it this way. But the end result is that lots of dedicated public servants who worked hard for modest paychecks are leaving their offices in a hurry, and feeling slighted. They deserve better. They deserved to be treated like the U.S. Attorney for the Northern District of New York.

    Statutes of limitations are unfair and arbitrary. For the non-lawyers, statutes of limitations are time limits on your opportunity to bring a lawsuit. Some of them appear reasonable. For example, here in New York State, you have three years to bring a suit against a person or entity whose negligence, or carelessness, caused you to suffer an injury. In the case of medical malpractice, where a healthcare provider, such as a doctor, makes an error while treating you, and the error results in injury or death, you have two and one-half years to sue.

    But if that carelessness, or that medical error, took place in a city or state-run facility think NYCHHC facilities, such as Bellevue Hospital you have ninety (90) days to file a notice of claim against the appropriate city or state organization behind the healthcare facility, or you are forever barred from bringing a lawsuit, no matter how egregious the misconduct, and no matter how devastating the injuries, barring certain rare exceptions.

    Let me put that into context for you. I have a client who suffered severe injuries at his birth because a hospital run by New York State failed to take steps to address a neonatal blood disorder of which they were aware. Because the parents were quite sophisticated and smart, they realized the need to reach out to a lawyer quickly, and with my office s help, filed their notice of claim against New York State within ninety days, preserving their right to bring an action on behalf of their son, who will need care for the rest of his life. Between New York s Medical Indemnity Fund (MIF) and the state, the money for that family s needs will be there, thanks to the settlement being negotiated on the family s behalf.

    But then there is the case of the young lady I spoke with last week, who lost her baby a few weeks shy of his expected birth date due to atrociously poor neonatal care from a NYC hospital. A single mother with other children to care for, and with a background that included little education, she did not reach out for help until well after the ninety days had expired, and I couldn t help her. She had been trying to get back on her feet after losing a child, and simply had not gotten around to thinking more carefully about the suspicious-sounding explanations she had been given in response to her questions on how she lost her baby. The case, if there was a case, would have had a very limited value in terms of damages, for reasons not relevant to this discussion. But the time limitation here was not helpful to any party, and kept a potentially deserving one from accessing our courts.

    And perhaps you ve noticed that hospitals run by NYC or New York State do not alert you to the the ultra-short times facing you if you are unfortunate enough to be the victim of medical malpractice. Well, now you know. Tell your friends. Contact my office if you have any questions.

    Reflective Practice in Social Work — The Ethical Dimension

    Recently, I received an urgent voicemail message from a hospital social worker: “Please get back to me as soon as possible. I have a meeting tomorrow morning with our head of human resources, and I’m very nervous about it.”

    Later in the day, I connected with the social worker and learned the following: He had been employed by the hospital for seven years and had never been disciplined. His current predicament began when his immediate supervisor called him in to discuss concerns about possible boundary violations and an alleged inappropriate dual relationship with a hospital patient. The social worker explained to me that in his personal life he is actively involved in a community-based group of parents who adopted children from China. The group sponsors a wide range of activities to support and enhance the children’s ethnic identity. Through this involvement, the social worker said, he and his wife had become very friendly with several other adoptive parents.

    About three weeks earlier, one of the parents who had become a good friend was admitted to the social worker’s hospital for treatment of a chronic, debilitating infection. The friend did not receive social work services. During the friend’s hospital stay, the social worker occasionally stopped by his room to say hello and inquire about the friend’s health. The patient’s attending physician had collaborated professionally with the social worker in other hospital cases and was well aware of the patient’s friendship with the social worker.

    One afternoon during the patient’s hospital stay, the physician contacted the social worker and explained that the patient was distraught after having just learned that he was diagnosed with bone cancer. According to the social worker, the physician asked the social worker to visit the patient and offer emotional support. The social worker visited the patient in his room and spent about an hour helping his friend process the distressing medical news.

    The social worker documented this patient encounter in the hospital chart. During a random quality-control review of social workers’ chart entries, the hospital’s social work supervisor read the note and became concerned because the social worker had not been assigned to provide social work services to this patient. The supervisor learned of the social worker and patient’s friendship and notified the director of human resources, who documented this “incident” in the social worker’s personnel record and asked to meet with the social worker.

    The Nature of Reflective Practice
    In 1983, the late scholar Donald Schon published his influential and groundbreaking book The Reflective Practitioner: How Professionals Think in Action . Schon’s thesis, based on his extensive empirical research, was that the most skilled and effective professionals have the ability to pay critical attention to the way they conduct their work at the same time that they do their work. Schon coined the terms “knowing-in-action” and “reflection-in-action,” which suggest that some professionals can take a step back and think hard about what they are doing while they are doing it. The concepts are akin to the widely used social work concept “use of self.”

    Ordinarily the concepts of knowing-in-action and reflection-in-action are applied to practitioners’ cultivation and use of technical skill, whether in surgery, architecture, town planning, engineering, dentistry, or psychotherapy. In my view, and as the above case demonstrates, social workers would do well to extend the application of these compelling concepts to their identification and management of ethical issues in the profession. Ideally, effective practitioners would have the ability to recognize and address ethical issues and challenges as they arise in the immediate context of their work, not later when someone else points them out. Put another way, social workers would have a refined “ethics radar” that increases their ability to detect and respond to ethical issues.

    Of course, the most important benefit is client protection. However, an important by-product is self-protection, that is, the increased likelihood that social workers will protect themselves from ethics-related complaints.

    Implementing Reflective Ethics Practice
    Certainly the hospital social worker who called me with panic in his voice would have benefited from reflective ethics practice and highly sensitive ethics radar. Had he reflected on the ethical dimensions of the boundary challenges that emerged when he interacted with his friend and hospital patient, it is likely that this well-meaning practitioner would have avoided his unpleasant encounter with the human resources department. The social worker’s decision to visit his friend was not the error; that was a humane and compassionate gesture. The error, rather, was not reflecting on his role in that moment and managing the boundaries carefully, including discussing them with his friend and his supervisor.

    In my experience, ethics-related reflection-in-action entails three key elements.

    Knowledge: Skillful management of many ethical dilemmas requires knowledge of core concepts and prevailing standards. Ethics concepts are addressed in professional literature and standards exist in several forms, including relevant codes of ethics, agency policies, statutes, and regulations. For example, the National Association of Social Workers’ Codeof Ethics includes explicit standards pertaining to boundaries, dual relationships, and conflicts of interest (especially section 1.06). It would have been best for the hospital-based social worker to consult relevant literature and standards with regard to conflicts that can arise when a social worker encounters a friend or social acquaintance in the work setting. The hospital’s personnel policies also prohibit dual relationships that involve conflicts of interest.

    In some cases, although not all, statutes and regulations address ethical issues. In the United States, both federal and state laws address various ethical issues, such as confidentiality, privileged communication, informed consent, and social workers’ ethical conduct. Such laws would not have been particularly helpful in the hospital social worker’s case, but often they are helpful and critically important, for example, when social workers must decide whether to disclose confidential information without clients’ consent to protect a third party from harm or whether parental consent is necessary to provide services to minors who seek help with substance abuse but insist that this information be withheld from their parents.

    Transparency: Reflective social workers who sense an ethical issue share their concern with supervisors, colleagues, and appropriate administrators. An effective way to protect clients and practitioners alike is to avoid any suggestion that the ethical issue is being handled “in the dark.” Such clarity demonstrates social workers’ good faith efforts to manage ethical dilemmas responsibly. When appropriate, clients should be included in the conversation.

    Process: Although some ethical decisions are clear-cut, many are not. The hospital social worker who contacted me was unsure about the best way to manage his involvement with a good friend who had become a patient. Unfortunately, the social worker did not notify his supervisor about the dilemma or seek consultation. He documented his lengthy hospital-room encounter with the patient, but doing so in the client’s hospital chart created the impression that the social worker was functioning in his professional capacity, not as a friend. My hunch is that had the social worker notified his supervisor of his friendship with the patient and made clear that any contact with the patient occurred as a friend, the social worker may have avoided any adverse personnel issues. What I have learned is that many ethical decisions are not simple events; they require a considerable, often painstaking, process.

    During the course of the profession’s history, social workers have refined the art of reflective practice. Historically, these skills have been applied primarily to clinical, policy, advocacy, and administrative functions. Clearly, reflective practice should extend to ethics as well.

    Medical coders work in the billing office, or “back office” of medical practices or hospitals. Medical coders help to complete, review, and process medical claims to help physician practices and hospitals get reimbursed from insurance companies for services and facilities provided to patients.

    Each medical procedure and patient encounter has a number, or CPT code, associated with it which corresponds to another code for a diagnosis (an ICD code).

    This helps insurance companies account for and track the money they reimburse to physicians and practices, to help prevent fraudulent medical claims or errors in payment.

    Skills Required

    Medical coders need to work well with numbers, and also should have a great attention to detail. While much of the job entails administrative tasks such as reviewing, processing, and submitting medical claims, some interpersonal skills are necessary. At times the medical coder may have to obtain additional information from a physician or other medical provider in the office. Also, the medical coder may have to contact insurance companies regarding questions about claims.

    Medical coders should also be comfortable working with computers and various types of medical coding and billing software programs.

    Educational Requirements and Professional Certifications

    To become a medical biller or coder, you must have a high school diploma and pass an accredited program in medical coding.

    In Practice

    The American Academy of Professional Coders (AAPC) offers accredited certification courses to achieve a CPC certification (Certified Professional Coder), and other related certifications for medical coders.     An accredited course should cost between $1,000 and $2,000.

    The academy recommends completion of a medical terminology course and anatomy course prior to taking the medical coding course for certification.

    Compensation for Certified Professional Coders

    There is a great deal of range and variance among coding salaries. This could be due to the fact that many coders work part-time or on a contract basis, as billing and coding needs often fluctuate throughout the year for a hospital or medical practice.

    For example, Indeed.com states that coding salaries range from $22,000 to $67,000 annually, in a salary search conducted in 2016. Typically, the coders with the CPC certification earn a great deal more than the non-certified coders. Also, as mentioned previously, the number of hours worked makes a huge difference in pay. Coders working full-time and/or overtime earn much more. Also, based on Indeed.com s job titles and salary examples, it appears that hospital-based coders (inpatient coders) earn more than office-based or outpatient coders.

    According to the most recent salary survey from the AAPC, the average salary for employed coders was $50,775, an 8 percent increase from the previous year.

    Also, the AAPC survey found that salaries increased proportionately with education and experience. Their survey also found that inpatient billers earn more than outpatient billers typically.

    What s to Like

    There is a lot to love about a career as a medical coder. Medical coders should continue to be in high-demand throughout the future of healthcare. Even though software has been developed to assist with the role, a qualified person is needed to enter the data and follow up on any discrepancies between the health care provider (physician), the hospital, and the insurance company. An effective and efficient coder is essential to the financial success of a practice.

    The coders help the physicians to get reimbursed accurately and on time. The hours for medical coders are fairly set and standard; coders don t have to take call or work nights or weekends. A 40-hour work week, Monday through Friday, is standard.

    Additionally, medical coding can be an excellent stepping stone into a more lucrative, higher-level career in medical office management or medical office operations, if you wish to earn more or do more in the long-term beyond medical coding.

    Medical coders do not have much interaction with patients, except at times by phone or at check-out.     Definition: Legal transcription is converting audio dictation by legal professionals and other recordings from legal cases into the printed word. Like medical transcription, which transcribes a physician’s dictated notes, this type of transcription requires specialized knowledge of terms used in the field. However, unlike medical transcription, formal certification is not required, but experience and/or education in the legal profession, as well as fast and accurate typing, are essential.

    The. MORE types of material a legal transcriptionist might transcribe include recordings of hearings, interviews, and depositions; dictation by legal professionals, and sometimes written documents such as handwritten communications, notes or other legal documents.

    Legal transcription is not the same as court reporting. Court reporting is a form of realtime transcription, meaning words are transcribed as they are spoken live—not from a recording. Court reporters must undergo certification.

    Work-at-home angle: Legal transcription is frequently done from home. Legal transcriptions may work for legal services companies, government agency or law firms as employees or as independent contractors. Like most work-at-home positions, employers and clients usually desire experience working on-site before allowing a home-based work.

    Real-Time Transcription

    Definition: Real-time transcription describes transcription that uses real-time text (RTT) technology to transcribe oral language as it is being spoken. Other forms of transcription work with audio recordings, rather than live speech. The most common use of real-time transcription is court reporting; however, other forms of it include real-time captioning and communication access real-time translation (CART), which is real-time transcription for the deaf.

    Real-time writers need to be able to type. MORE a 200-300 wpm. They must have excellent hearing and listening skills, knowledge of spelling, punctuation, and grammar. Additionally, anyone practicing real-time transcription will need to be detail oriented and able to think fast and concentrate for long periods. Training in stenography and stenographic software is needed as well.

    To gain the skills needed to be a real-time writer, post-secondary schooling is needed, and most jurisdictions require certification for court reporters, so like medical transcriptionists, court reporters, and other real time writers will need to enroll in training and/or certification classes. For more information on these certifications, see the National Court Reporting Association’s (NCRA) website. which offers information about both court reporting and captioning training.

    Work-at-home angle: Typically court reporting is performed in person in courtrooms and at depositions, and CART work is also done on site, though some forms may be done remotely.

    However, those with realtime writing skills and experience can use them in work-at-home settings by transitioning to captioning. However, to become a captioner, additional training and possibly certification may be required.

    Medical Trancriptionist

    Definition: A medical transcriptionist practices a specialized form of transcription. He or she listen to a physician or medical practitioner’s dictated notes regarding a patient and transcribes them so they can be added into the patient’s medical file. Typically a medical transcriptionist uses similar equipment to a general transcriber s computer. This includes a headset, foot pedal, and specialized transcription software.

    Unlike most other forms of transcription, medical transcription. MORE requires post-secondary training–either a 1-year certificate program or 2-year associate’s degree. The types of courses required in these programs include anatomy, medical terminology, medical legal issues, and grammar and punctuation.

    There are two types of certifications: registered medical transcriptionist (RMT) and certified medical transcriptionist (CMT). Certifications require an initial exam and then retesting periodically and/or continuing education.

    General skills needed to be a medical transcriptionist:

    • Fast and accurate typing
    • Detail-oriented, careful worker
    • Knowledge of medical terminology
    • Knowledge of medical transcription practices
    • Excellent English grammar, punctuation, and style
    • Ability to work independently and under time pressure
    • Excellent hearing and listening skills

    Work-at-home angle: Medical transcriptionists often work at home. However, usually only experienced medical transcriptionists get this opportunity. The new medical transcriptionist will likely need to work in an office before working at home.

    Medical transcriptionists may be employees or independent contractors. As independent contractors, they might run their own home business and work directly with medical professionals or work for a medical BPO or other company that hires medical transcriptionists as contractors.

    Bachelor s Degree

    Students can obtain either of Bachelor of Arts in Art, Graphic Design or a related field. Many schools offer a Bachelor of Fine Arts (BFA). Note that a combination of online and brick-and-mortar classes may be necessary for a BFA.

    Master s Degree

    Candidates for these programs typically should have either a bachelor s degree in art or in a related field, such as Studio Art, Communications, Theater, Media Studies, Multimedia Communications. Many people have a Bachelor of Fine Arts (BFA) degree.

    A variety of master s degrees in art are offered. In the Master of Arts (MA). students pursue concentrations in order to become an expert in a specific discipline. A Master of Fine Arts (MFA) is considered a terminal degree (the most advanced) and will qualify students to teach art at a college level. Most MFA programs have portfolio requirements.

    Common areas of focus for an MA offered online include:

    • Art History tracks the evolution of Eastern and Western art forms. You will develop your skills combining key parts of different art genres to create original, compelling works.
    • Creative Writing focuses on specific disciplines and study prominent examples of great fiction. Expect to dissect narratives, trace literary motifs and discuss characterizations in order to refine your craft.
    • Art Education teaches students to build curricula that promote exercises including short story composition and illustration, which will help students grasp artistic concepts such as theme and characterization. You may also identify and develop your unique form of visual or written expression.
    • Music Education examines how musical theories and works differ among diverse cultures. This specialization focuses on how to develop students’ musical competencies from a technical standpoint at early grade levels or to teach more complex musical concepts such as combining music genres and lyricism at the post-secondary level.
    • Media Design teaches brand development through the use of broadcasting and social media tools. Students will evaluate promotional tools used by individual artists as well as large business enterprises including social media and viral marketing.

    Coursework

    Students at the undergraduate and graduate level can expect courses in:

    • Art and Design History. Students gain an understanding of the practices, criticisms and societal implications of art. Major art movements such as Realism, Impressionism and Cubism are discussed as well as the emergence of graphic design in the twentieth century.
    • Studio Practices. Students concentrate on best practices for producing art in a studio. This involves hands-on work, such as creating storyboards for animation or preparing plates for printmaking.
    • Graphic Design: Students experiment with theories of design and their applications in commercial settings. Students learn about page layout, color theory and typography.
    • Color Theory: Students learn how to manipulate color in their work to produce a variety of visual effects.
    • Criticism: Students learn how to generate new ideas and perspectives while analyzing, investigating and seeking meaning behind works of art and design.
    • Drawing: Drawing classes cover a wide range of styles and teach students to translate 3D space onto a 2D surface using texture, depth, line and value.
    • Art Marketing: Students will analyze PR strategies, media relations and social networking tools. They will learn how to clearly promote the content of their art and generate public anticipation using new viral marketing techniques and mobile technology.
    • Art and Public Policy: This course introduces students to different public institutions and policies that protect artistic expression and provides a promotional platform for specific works. They will learn about the processes and politics surrounding ratings boards, location scouting and the creation of public art installations.

    Careers

    Students pursuing an art degree often seek work in advertising, film production, television production and interior design. All of these industries need art directors, animators web designers, graphic designers and fine artists with bachelor’s degrees. Although accreditation is not mandatory, a company is more likely to favor a job candidate with a degree from an accredited program. Students interested in graduate school should know that jobs that require a graduate degree in art are mostly in the museum and education fields.

    A student with an online masters degree in art can become an art instructor in higher education, an art historian or a curator in a museum. To become an art professor or museum director, students should pursue an advanced degree.

    Some careers graduates can choose from include:

    • Art Directors: Art directors work with creative departments and clients to develop the visual style of films, magazines, newspapers and products. The median annual wage for this profession is approximately $80,600. By 2020, this field is expected to grow by 9 percent as less directors are needed for print publications and more are needed for digital media and graphic design firms.
    • Museum Curators: Museum curators acquire, store, trade and display large collections of art and historic artifacts. The median annual salary is roughly $42,300. Over the next ten years, this field will grow by 25 percent since museum attendance is expected to grow and more curators will be needed to coordinate renovation and restoration projects.
    • Post-secondary Art Teachers: Postsecondary art teachers teach a variety of artistic disciplines – including drama, music and graphic design – to college students. The median annual salary for this profession is approximately $62,300. Over the next decade, this field is expected to grow between 10 and 19 percent, as postsecondary enrollment is expected to increase and more aspiring artists turn to a formal education to further train in their craft.

    Radiology being transformed by cloud, proponents say

    If digital cloud technology proponents have their way, the field of radiology will be transformed, with major changes in who does the work of reading images, and where and how it’s done.

    Instead of storing images on local IT infrastructure and sharing images through physical means such as compact discs, cloud radiology allows storage and transmission of images through an off-site network of computer servers. This model makes it easier to back up files and access those files through multiple devices, without the fear of losing the data if an individual device is compromised.

    Experts say that will enable hospitals, medical groups and health IT firms to organize radiology practice in different ways that reduce costs and improve quality. It may mean more hospitals outsourcing the reading of images to external networks of radiologists or other specialists. The turnaround time on reading images may be significantly shortened. And shipping costs and maintenance costs for local storage infrastructure could be reduced.

    Increased efficiency may mean increased competition, fewer scans and fewer radiology jobs. A growing number of companies are entering this business and offering cloud-based radiology services to hospitals.

    If you went to the major radiology trade shows three years ago, you would hear, ‘I’m not sure about this cloud stuff,’ said Morris Panner, CEO of Phoenix-based startup DICOM Grid. But now, market penetration is rapidly accelerating. His firm claims 85 clients, including the Mayo Clinic. for its cloud infrastructure service.

    Dr. David Hirschorn, director of radiology informatics at Staten Island (N.Y.) University Hospital and a member of the American College of Radiology’s informatics committee, said the cloud may be the best way to create a central repository for images allowing doctors to look at the history of patients’ exams, which is critical for enabling doctors to spot significant changes in their conditions. Otherwise, doctors have to query hospitals for past images one-by-one.

    Hirschorn said that while cloud technology has been available for at least a decade, pressures such as rules requiring meaningful use of electronic health records have pushed providers to share more images, giving cloud-based radiology a boost.

    Dr. Jonas Rydberg, a professor of clinical radiology at the Indiana University School of Medicine, wants to eliminate the compact disc as a mode of sharing images and replace it with cloud-based sharing. Since 2012, Rydberg has been developing a cloud radiology network across Indiana, called IU Health Radiology Cloud. Currently, there are 79 participating hospitals in Indiana, along with a few in Illinois, and he hopes to add 25 more. He envisions a system in which providers can perform full radiological evaluations within a couple of hours versus the days it previously took to retrieve old images from other hospitals.

    In the past, Rydberg said, providers often used CDs in transmitting images. But CDs can be lost. In addition, he estimates that up to 5% of the images were unreadable because of corrupted discs, requiring retests. The cloud allows doctors to reach into the original picture archiving and communication system, or PACS, and access the exam, nearly eliminating the problems of lost or corrupted images, he said.

    MH Takeaways Storing and sharing images through the cloud may enable providers to speed up the reading of images, reduce staffing and infrastructure costs, do fewer scans, and spare patients from duplicative tests.

    A company called Imaging Advantage, based in Santa Monica, Calif. is using cloud technology to create a nationwide network of radiologists and claims 200 hospitals and health systems as clients. It partners with radiology groups. Naseer Hashim, the firm’s CEO, said when a client hospital requests a read of a scan, his firm can direct the read through the cloud to the right doctor in its network, for instance a radiologist specializing in neurological scans.

    Missing exams and problems in sharing images are common nationwide. A survey of 1,900 medical consumers recently released by the Office of the National Coordinator for Health Information Technology found that 18% of respondents had to hand-carry an X-ray, MRI or previous test to an appointment, and 6% had to redo a test because the previous results weren’t available. Using cloud-based systems, doctors are better able to easily access scans instead of requiring another scan.

    Rydberg said his cloud-based radiology project also improves quality of care. For example, waiting physicians can look at scans while a trauma patient is en route to the hospital, and the patient can be sent directly to the operating room. Doctors also can view images through the cloud and offer consultations remotely.

    In one case he cited, a trauma surgeon averted a helicopter airlift of a patient because he could tell from images he read over the cloud that the patient’s condition did not justify the airlift.

    One challenge to expanding cloud-based radiology is resistance from some physicians and hospitals who see this as taking away their patients. I regard this more as a psychological barrier than a rational barrier, Rydberg said.

    Hirschorn agreed that some providers worry that the greater efficiency of cloud-based image sharing means fewer scans and less business. He criticized institutional resistance to sharing images, saying that means more patients undergoing duplicative scans and being exposed to more radiation.

    It’s insane, it’s outrageous, and it’s inexcusable, he said.

    Still, he said that while cloud technology makes it easier to share data among institutions that pool data, hurdles remain in sharing with other institutions.

    30-minute turnaround

    The advantage for client hospitals, he said, is that if their staff radiologists are busy, Imaging Advantage can direct the read off-site to a radiologist affiliated with another hospital who is free at that particular time. If a hospital wants a scan read by a subspecialist, the company can do that, too. The company’s software tracks which radiologists have the appropriate credentials and state licensures.

    Hashim said his company’s system can help smaller hospitals that don’t have board-certified specialists in every area.

    By reorganizing the work and making radiologists more efficient, Hashim said, the company averages under 30 minutes for turnaround on reading a scan. And the ability for radiologists to do more reads means that productive doctors can get paid more. One obstacle to growth, he said, is that people still hold on to this conception that we need to have radiologists physically located in hospitals, he said.

    Dr. Arthur Radow, a Phoenix-area radiologist and past president of the Arizona Radiological Society, said his radiology group joined Imaging Advantage in 2011. Under Imaging Advantage, the group has dropped from 18 doctors to about 12, and the remaining doctors are 60% to 80% more productive and are making more money, he said.

    Georgia Medical Malpractice Laws Statutory Rules

    If you think you might have a viable medical malpractice case in Georgia, you’re likely wondering about the different state laws that might affect your injury claim. In this article, we’ll discuss a few key Georgia laws, including the “statute of limitations” for starting a medical malpractice case in the state’s civil court system.

    Statutes of Limitations and Award Limits

    The “statute of limitations” is the legal term for the amount of time that an injured patient (the plaintiff) has to file a medical malpractice lawsuit against a health care provider.

    Every state has a statute of limitations for medical malpractice cases, and these laws usually include a few exceptions and special rules that can complicate things. But the bottom line is this: If you don’t get your medical malpractice lawsuit filed before the applicable deadline set out in the statute, the court is almost certainly going to throw out your case without hearing it .

    Medical malpractice damage awards — in other words, how much money an injured plaintiff can receive in a lawsuit — are also limited or “capped” in some states. Both the strict statute of limitations and the damage caps are the result of states’ efforts to lower the cost of medical malpractice liability insurance.

    The rules affecting all of these issues in Georgia are discussed below.

    Standard Time Limit

    In Georgia, you must start the lawsuit within two years of the injury or death caused by the malpractice or, at the latest, five years from when the malpractice occurred. In other words, even if the injury didn’t somehow show up until five years after the act of malpractice, the case will be thrown out if you sue the health care provider more than five years after he or she caused the injury.

    Exceptions

    One exception is for the care provider’s fraudulent concealment of the malpractice, i.e. intentionally deceiving you so you don’t discover the malpractice. In that event, the statute of limitations is “tolled”, i.e. put on pause, until the malpractice is discovered.

    Another exception is for foreign objects left in the patient, for example a surgical sponge. If that happens, the plaintiff can sue within one year of discovering an object was left behind, regardless of whether the two or five year limitation periods have already passed.

    If the malpractice occurred before the plaintiff was five years old, the two and five year statutes of limitation begin when the plaintiff turns five.

    If someone is mentally disabled, the two-year statute of limitations is tolled if and until they recover, but the overall five year statute of limitations still applies.

    The basic laws for Georgia Medical Malpractice Statute of Limitations can be found starting at O.C.G.A. § 9-3-71.

    The “Expert Affidavit” Requirement in Georgia

    Along with the complaint — that’s the document that starts the lawsuit — the plaintiff in a Georgia medical malpractice case must file an affidavit prepared by a medical expert who is qualified to testify under Georgia law, and who can provide evidence of at least one negligent act committed by the defendant health care provider, including the factual basis for that allegation.

    It’s possible to get a 45-day extension to file this affidavit, but only in rare situations where the lawsuit filing deadline is right around the corner and you’ve only recently hired your attorney, so that there wasn’t reasonable time to prepare and file the affidavit.

    No Damage Caps

    A number of states have enacted caps on medical malpractice damages, including limitations on the amount of non-economic damages (such as those meant to compensate for “pain and suffering”) that an injured patient can recover.

    Georgia previously had damage caps that, among other things, limited a medical malpractice plaintiff’s total recovery of non-economic damages from one defendant to $350,000 — and recovery of these damages from all defendants to $1,050,000. However, these caps were ruled unconstitutional by the Georgia Supreme Court in 2010 and are no longer in effect .

    In Georgia, punitive damages are limited to $250,000 in medical malpractice cases (except in extremely rare cases where the plaintiff can show an intent to harm on the part of the defendant).

    Medical careers are good paying, fast growing, and in demand. Whether you are a recent graduate or looking for a list of different career choices, you have come to the right place.

    The health care professions combine technology and the human touch to address the needs of patients.

    All the different medical careers on this site are professional workers, making sure their patients are cared for 24 hours a day, seven days a week. The healthcare industry is large, with more than 14 million jobs in the U.S. alone.

    But there are many types of health care professions, with varying education requirements.

    We will look at medical careers that require 2-year degrees, 4-year degrees, and 7-year degrees on this site.

    I created a list of medical careers when I considered a career in health care.

    Each of the health care professions require a minimum of an post-secondary degree.

    Each also has the ability for further educational opportunities, including receiving a master’s degree, which was important to me for future growth in the position.

    They are also good paying medical careers.

    I also created a master list of medical careers. which has more than 40 health care professions.

    Although you can get started in the health sector in less than 6 months, you will be better served to identify which medical career is right for me.

    ‘What medical career is right for me?’

    A career in the medical field provides many incentives. It is important to identify traits about yourself, which I’ll try to help you do over the next few paragraphs.

    Are you a hermit?

    You have to interact with people and help them on a daily basis. But don’t let that scare you, especially if you are not social by nature.

    Many fields in medicine — such as a laboratory technologist — don’t require patient interaction.

    Interaction is an important trait you need to consider for your career. You will want to ask yourself — am I a hermit? am I social? am I somewhere between these two extremes.

    For example, I like time alone to research and make decisions, but I also like interaction.

    I knew a position such as a lab tech would not suit me because I’d drive myself crazy being left by myself. At the same time, I knew a position as a nurse would not suit me. either, because of the massive amount of patient interaction.

    This is the thought process you should consider. You need to learn about yourself when picking a medical career.

    One solution is to talk to a career counselor, who may be able to suggest you take a personality test like the Myers-Briggs.

    This test will offer insight about yourself and provide different career choices.

    Also, your workplace location will be influenced by the medical career you choose.

    Many in the health sector work in hospitals, physician offices, cancer centers, substance abuse facilities, laboratories, and research facilities.

    How secure is my job?

    Job security can be important. We want to know the career path we are picking will have jobs in it in the future.

    With health care professions, that is the case. Medicine relies on people needing care — and there’s no shortage.

    With baby boomers starting to reach retirement age, there will be an increased need for health workers. Also, in many cities the major employer is the hospital. This is important for three reasons.

    Stability: The likability of the hospital closing up shop is non-existent.

    Relocation: You can move to just about any state, get licensed, and be able to find a job easily.

    Advancement: At a large organization such as a hospital, you have many different medical careers that you could experience and have the opportunity to be hired for.

    Job outlook and the BLS

    Related to security is job outlook. The Bureau of Labor Statistics offers data on job outlook. (Note: This data was from 2013 when this was written.)

    The data can be misleading, especially when you look at rate of growth. An example is nuclear medicine technologists.

    The BLS notes that there will be a need for 20 percent more nuclear medicine technicians by 2020. That sounds like a lot, but it is not and you have to look at the data closer.

    In 2013, there are 16,900 technicians who handle nuclear medicine procedures. Twenty percent of 16,900 is 3,400.

    There will be less than 3,500 more of these technicians needed. Despite 20 percent growth, this likability of finding a job in this position after graduation may be tough.

    Compare that to nursing. The BLS notes there will be a need for 20 percent more registered nurses by 2020.

    Currently, there are 2,737,400 RNs. Twenty-six percent of this is 711,900. Become a nurse and you won’t have a problem finding a job.

    The point: Percentages can be misleading. Please do yourself a favor and check expected growth in numbers, not percentages.

    High-paying medical careers

    You want to get paid, I’m sure. Medical careers pay well, but you should not just look at dollar signs.

    From a money standpoint, a radiation technologist and nuclear medicine technician have the highest median pay — 74,980 and $68,560, respectively — and low education requirements.

    Travel medical insurance: Don’t leave without it

    During six months in Chile while interning for a nonprofit organization, a 20-something man we’ll call Lucas contracted scabies. The microscopic mite infestation drove Lucas — who asked that we not use his real name — to seek treatment at a free local clinic.

    Before leaving the U.S. Lucas had opted against purchasing supplemental travel medical insurance, which would have covered his care at a hospital. Like many people traveling abroad, he chose to play the medical odds.

    Insurance for health catastrophes overseas

    A twisted ankle, common cold or routine aches and pains might be addressed in a foreign land with the help of a local pharmacy and perhaps a good dictionary in the native language.

    But medical emergencies aren’t always so clear-cut overseas, says Steve Wyss, managing director of Affinity Group Underwriters, a provider of insurance to professional associations and other groups.

    “It’s when you have something catastrophic, or need to be moved to an area with a higher standard of care, that having a travel medical insurance policy can really make the difference,” he says.

    Different from travel insurance

    Many vacationers are familiar with travel insurance, which is usually offered while purchasing an airline ticket or cruise. Such policies can be helpful to recoup the costs of lost luggage or a change in plans.

    Travel medical insurance is somewhat different. It supplements your regular health insurance when you’re traveling abroad.

    While some people are willing to take chances, travelers with medical conditions or who engage in high-risk activities might want to make sure any surprise trips to an emergency room won’t crimp their finances long after they’ve returned home.

    Picks up where health insurance leaves off

    When you travel, your health insurance likely will offer basic coverage for emergencies outside the U.S.

    Some policies even have a clause for emergency evacuation (also known as “air ambulance”) that will reimburse a patient for the cost of a flight from a remote location to a hospital with a higher level of care, Wyss says.

    But your regular health insurance provider “probably doesn’t have a network outside the U.S.,” notes Brendan Sharkey, director of individual product sales for GeoBlue Travel Insurance, the travel health insurance arm of Blue Cross/Blue Shield. “This gets complicated not only when trying to find the best resources, but also when struggling with paperwork while filing a claim.”

    Travel health risks can be far from exotic

    As Lucas found out, the health risks for travelers can include more than unexpected medical emergencies such as deadly snakebites, serious crashes or malaria.

    Sharkey says the claims he and his colleagues see most frequently are for flare-ups of chronic health conditions.

    “Diabetes or asthma — those are the things that can spiral out of control when someone is traveling,” he says. “Someone with a heart condition, who’s dealing with sleep and dietary changes, more physical exertion than usual, walking all along hilly streets to discover a new city — sometimes people will push themselves too far.”

    Travel medical insurance for pregnancy

    The birth of Sharkey’s son in Australia several years ago is a perfect example of a fairly ordinary medical issue that can become critical. Sharkey hadn’t yet started working for GeoBlue, and the couple opted to use the public health care system during the pregnancy and delivery.

    “My wife’s roommate in the hospital was an inmate of the local prison, shackled to the bed with a guard standing over her,” he says.

    There was no epidural; a midwife alone began the delivery, and when complications arose, Sharkey had to track down the doctor who was overseeing five simultaneous deliveries.

    “Had we had insurance,” he says, “we would have had private care, and it would have been different than this white-knuckle ride.”

    Policies for less than $20 per week

    The costs of supplemental travel medical insurance policies can vary greatly, but are determined by the length and destination of the trip and the age of the traveler.

    Coverage can be fairly inexpensive — less than the cost of checking a bag.

    For example, a 40-something traveler could spend less than $20 per week for a policy with a $500 deductible that would provide up to $500,000 in medical insurance for accidents or illness and include medical evacuation coverage.

    Buy before you leave

    The time to buy a travel medical insurance policy is the moment travel reservations have been made and paid for. Don’t wait until you’re on the road, because those who attempt to buy a travel medical policy once they’ve already left home are more often than not wasting their money, Sharkey cautions.

    “If you’re a short-term traveler or backpacker looking to buy a policy last-minute because you’re already showing symptoms, it’s too late,” he says. “You’re going to be basically uninsurable.”

    The fine print on exclusions

    Some legitimate travel medical insurance policies won’t cover a traveler who is injured due to an act of terrorism, especially if that act occurs in a country with a State Department travel alert in place.

    Others will deny claims for a pre-existing condition, even if that condition has been asymptomatic, undiagnosed and untreated before the policy was purchased.

    Before you start shopping for travel medical insurance, call your existing health insurance company to determine if it includes any international coverage. If not, ask your insurer if it sells supplemental travel medical policies or if it partners with another provider that does.

    Beyond that, the U.S. State Department’s website maintains a list of travel medical insurance providers.

    The Health Information Management online associate degree program is accredited by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM). According to the Bureau of Labor Statistics, health information management jobs are expected to increase by 15% from 2014-2024. The median income is $37,110, (2015 statistic), but additional credentials do add to the marketability of professionals.

    The HIM program was established in 1998 as a traditional face-to-face program. In 2007-2008, the HIM program transitioned to an “online only” program to meet the needs of working students and professionals. The exception to online courses are the requirement for clinical courses completed onsite at a facility (HITT 2261 for the associate degree and HITT 1167 for Medical Coding Specialist certificate).

    Students must be self-motivated, able to comprehend information in written form, and have the ability to communicate effectively both in written and verbal forms. The courses are not self-paced, but have scheduled weekly due dates for assignments, discussions, and tests. Students should expect to spend 6 9 hours per 3 hour credit course to complete assignments, readings, and tests. Instructors are available to answer questions online or by phone related to course topics.

Most of the students in the programs are considered part-time; taking 2 to 3 classes each semester since most have part-time or full-time jobs. There are approximately 70-80 students taking HIM classes or prerequisite courses to gain admission to the program.

    Clinical Requirement Onsite:

    Students that live outside the local Midland area are able to locate clinical sites in their area with the assistance of the HIM program. More information and directions are given to students upon acceptance into the program (courses related to associate degree and Medical Coding Specialist certificate). Prior to the clinical course, students are required to meet certain facility requirements to attend clinical practice. They support patients – serving-one-on-one – sometimes even as an extended family ‘coach’ during therapy and group activities when needed.     Course 5: Medical Billing Insurance Claims Process

    The medical billing insurance claims process starts when a healthcare provider treats a patient and sends a bill of services provided to a designated payer, which is usually a health insurance company. The payer then evaluates the claim based on a number of factors, determining which, if any, services it will reimburse.

    Let’s briefly review the steps of the medical billing procedure leading up to the transmission of an insurance claim. When a patient receives services from a licensed provider, these services are recorded and assigned appropriate codes by the medical coder. ICD codes are used for diagnoses, while CPT codes are used for various treatments. The summary of services, communicated through these code sets, make up the bill. Patient demographic data and insurance information are added to the bill, and the claim is ready to be processed.

    Processing Claims

    A number of technical protocols and industry standards must be met for insurance claims to be delivered expediently and accurately between medical practice and payer.

    Medical billing specialists typically use software to record patient data, prepare claims, and submit them to the appropriate party, but there isn’t a universal software application that all healthcare providers and insurance companies use. Even so, insurance claims software use a set of standards, mandated as by the HIPAA Transactions and Code Set Rule (TCS). Adopted in 2003, the TCS is defined by the Accredited Standards Committee (ACS X12), which is a body tasked with standardizing electronic information exchanges in the healthcare industry.

    There are two different methods used to deliver insurance claims to the payer: manually (on paper) and electronically. The majority of healthcare providers and insurance companies prefer electronic claim systems. They are faster, more accurate, and are cheaper to process (electronic systems save around $3 per claim). But because paper claims have not yet been completely removed from the insurance claims process, it is important for the medical biller and coder to be well versed with both electronic and hardcopy claims.

    Filing Electronic Claims

    Certain technologies have been introduced into the system in order to expedite claim processing and increase accuracy.

    Some healthcare providers use software to electronically enter information into CMS-1500 and UB-04 documents. Using “fill and print” software eliminates the possibility for unreadable information. This software may also include certain types of “scrubbing,” or tools that check for errors in the documents. While these tools do decrease the amount of errors made in filling out claim forms, they are not always 100 percent accurate, so medical billers should remain diligent when filling out forms using software.

    Optical Character Recognition (OCR)

    OCR equipment scans official documents, electronically isolating and recording information provided in the different fields, and transferring (or auto-filling) that information into other documents when necessary. While OCR technology helps make hardcopy claim processing much more efficient, human oversight is still needed to ensure accuracy. For instance, if the OCR miscalculates a simple digit in a medical code, that error must be flagged and manually corrected by a medical billing specialist.

    Note that when OCR equipment is not available, it is possible for a medical billing specialist to manually convert CMS-1500 and UB-04 documents into digital form using conversion tools called “crosswalks” (note that the same term applies for tools used to convert ICD-9-CM codes to ICD-10-CM). You can find crosswalk references from a number of different sources.

    Filing Manual Claims

    Paper claims must be printed out, completed by hand, and physically mailed to payers. The healthcare industry uses two forms to submit claims manually. Since processing paper claims requires more manual interaction with forms and data, the opportunity for human error increases compared to electronic claims. Documents can be printed improperly, and handwritten codes can be incorrect or illegible. The forms can also be mailed to the wrong address, with insufficient postage, or disrupted by logistical complications with the delivery services. These errors are costly for the healthcare provider, often resulting in form resubmission (a time-consuming process) and payment delays.

    Generally, healthcare professionals like family physicians use form CMS-1500, while hospitals and other “facility” providers use the UB-04 form.

    The CMS-1500 is the universal claim form used by non-institutional healthcare providers (private practices, etc.) to bill Medicare for Part B covered services and some Medicaid-covered services, and is accepted by most health insurance providers. The CMS-1500 is maintained by the National Uniform Claim Committee (NUCC), and was previously updated to include National Provider Identifiers (NPIs), or unique numbers required by the Health Insurance Portability and Accountability Act (HIPAA).

    Form CMS-1500 contains all the basic information needed to submit an accurate claim. This includes fields for the patient’s demographic information, insurance information, and boxes in which to provide medical codes and corresponding dates of service. Certain boxes are used exclusively for Medicare and/or Medicaid. It is important to note that different payers may provide different instructions on how to complete certain item numbers. The medical biller and coder should be familiar with specific payer requirements before filling out the form.

    Form UB-04, also maintained by the NUCC, is very similar to the CMS-1500, but it is used by institutional healthcare providers, such as hospitals. Like the CMS-1500, the UB-04 is used in lieu of electronic claims when the facility meets any number of exceptions granted by the ASCA. It is the responsibility of the facility to self-assess whether these designated exceptions apply to their operation, granting usage of manual claims. Also similar to the CMS-1500, certain payers may not require all fields, or data elements, to be completed.

    The role of clearinghouses

    Once a file is created using these standards, it is usually sent off to a clearinghouse. The clearinghouse is a third-party operation that primarily acts as a middleman between healthcare providers and insurance carriers.

    Think of the clearinghouse as a central hub, or a single location where all claims are sent to be sorted and directed onward to all the various insurance carriers. Typically, clearinghouses use internal software to receive claims from healthcare providers, scrub them for errors, format them correctly in accordance with HIPAA and insurance standards, and send them to the appropriate parties. Clearinghouses generally keep medical practices in the loop during this process by providing reports on the status of claims.

    This third party is necessary because healthcare providers typically have to send high quantities of insurance claims each day to a variety of different insurance providers. Each of these insurance providers may have their own submission standards. If a medical practice’s billing staff was solely responsible for transmitting insurance claims under both insurance and HIPAA requirements, the potential for error would increase dramatically, not to mention the time required for formatting each claim to specific insurance carrier.

    When choosing a clearinghouse, a healthcare provider should consider two main factors:

    • Does the clearinghouse have the capability to work with the insurance providers the practice works with most often?
    • Can the clearinghouse accommodate claims transmissions from the insurance provider’s practice management software?

    Confirming these questions ensures that all transmissions run smoothly.

    Wrapping Up Course 5

    Healthcare providers prepare insurance claims using information provided in the patient’s bill. Occasionally, the claim is prepared manually and sent by mail. In most cases, the claim is sent electronically (having either been prepared using claim software or scanned from a hard copy) to a clearinghouse. The clearinghouse checks the claim for errors, formats it according to HIPAA and insurance guidelines, then transmits it to the appropriate payer, while also sending a report back to the healthcare provider.

    After the claim has been evaluated, the insurer must provide both the patient and healthcare provider with an Explanation of Benefits (EOB). The EOB breaks down the adjudication process, showing the dates of service, procedures and charges, patient financial responsibility, and the amount paid to the healthcare provider. At this point, the health insurer sends payment to the healthcare provider, usually in the form of an electronic fund transfer.

    The insurance claims process can be complex. Fortunately, there are tools to help complete insurance claims on a day-to-day basis. Medical billing professionals who are familiar with these tools and all documents (both paper and electronic), industry standards, individual insurance company regulations, clearinghouse procedures, and the adjudication process will be prepared to succeed.
