Radiology being transformed by cloud, proponents say

If digital cloud technology proponents have their way, the field of radiology will be transformed, with major changes in who does the work of reading images, and where and how it’s done.

Instead of storing images on local IT infrastructure and sharing images through physical means such as compact discs, cloud radiology allows storage and transmission of images through an off-site network of computer servers. This model makes it easier to back up files and access those files through multiple devices, without the fear of losing the data if an individual device is compromised.

Experts say that will enable hospitals, medical groups and health IT firms to organize radiology practice in different ways that reduce costs and improve quality. It may mean more hospitals outsourcing the reading of images to external networks of radiologists or other specialists. The turnaround time on reading images may be significantly shortened. And shipping costs and maintenance costs for local storage infrastructure could be reduced.

Increased efficiency may mean increased competition, fewer scans and fewer radiology jobs. A growing number of companies are entering this business and offering cloud-based radiology services to hospitals.

If you went to the major radiology trade shows three years ago, you would hear, ‘I’m not sure about this cloud stuff,’ said Morris Panner, CEO of Phoenix-based startup DICOM Grid. But now, market penetration is rapidly accelerating. His firm claims 85 clients, including the Mayo Clinic. for its cloud infrastructure service.

Dr. David Hirschorn, director of radiology informatics at Staten Island (N.Y.) University Hospital and a member of the American College of Radiology’s informatics committee, said the cloud may be the best way to create a central repository for images allowing doctors to look at the history of patients’ exams, which is critical for enabling doctors to spot significant changes in their conditions. Otherwise, doctors have to query hospitals for past images one-by-one.

Hirschorn said that while cloud technology has been available for at least a decade, pressures such as rules requiring meaningful use of electronic health records have pushed providers to share more images, giving cloud-based radiology a boost.

Dr. Jonas Rydberg, a professor of clinical radiology at the Indiana University School of Medicine, wants to eliminate the compact disc as a mode of sharing images and replace it with cloud-based sharing. Since 2012, Rydberg has been developing a cloud radiology network across Indiana, called IU Health Radiology Cloud. Currently, there are 79 participating hospitals in Indiana, along with a few in Illinois, and he hopes to add 25 more. He envisions a system in which providers can perform full radiological evaluations within a couple of hours versus the days it previously took to retrieve old images from other hospitals.

In the past, Rydberg said, providers often used CDs in transmitting images. But CDs can be lost. In addition, he estimates that up to 5% of the images were unreadable because of corrupted discs, requiring retests. The cloud allows doctors to reach into the original picture archiving and communication system, or PACS, and access the exam, nearly eliminating the problems of lost or corrupted images, he said.

MH Takeaways Storing and sharing images through the cloud may enable providers to speed up the reading of images, reduce staffing and infrastructure costs, do fewer scans, and spare patients from duplicative tests.

A company called Imaging Advantage, based in Santa Monica, Calif. is using cloud technology to create a nationwide network of radiologists and claims 200 hospitals and health systems as clients. It partners with radiology groups. Naseer Hashim, the firm’s CEO, said when a client hospital requests a read of a scan, his firm can direct the read through the cloud to the right doctor in its network, for instance a radiologist specializing in neurological scans.

Missing exams and problems in sharing images are common nationwide. A survey of 1,900 medical consumers recently released by the Office of the National Coordinator for Health Information Technology found that 18% of respondents had to hand-carry an X-ray, MRI or previous test to an appointment, and 6% had to redo a test because the previous results weren’t available. Using cloud-based systems, doctors are better able to easily access scans instead of requiring another scan.

Rydberg said his cloud-based radiology project also improves quality of care. For example, waiting physicians can look at scans while a trauma patient is en route to the hospital, and the patient can be sent directly to the operating room. Doctors also can view images through the cloud and offer consultations remotely.

In one case he cited, a trauma surgeon averted a helicopter airlift of a patient because he could tell from images he read over the cloud that the patient’s condition did not justify the airlift.

One challenge to expanding cloud-based radiology is resistance from some physicians and hospitals who see this as taking away their patients. I regard this more as a psychological barrier than a rational barrier, Rydberg said.

Hirschorn agreed that some providers worry that the greater efficiency of cloud-based image sharing means fewer scans and less business. He criticized institutional resistance to sharing images, saying that means more patients undergoing duplicative scans and being exposed to more radiation.

It’s insane, it’s outrageous, and it’s inexcusable, he said.

Still, he said that while cloud technology makes it easier to share data among institutions that pool data, hurdles remain in sharing with other institutions.

30-minute turnaround

The advantage for client hospitals, he said, is that if their staff radiologists are busy, Imaging Advantage can direct the read off-site to a radiologist affiliated with another hospital who is free at that particular time. If a hospital wants a scan read by a subspecialist, the company can do that, too. The company’s software tracks which radiologists have the appropriate credentials and state licensures.

Hashim said his company’s system can help smaller hospitals that don’t have board-certified specialists in every area.

By reorganizing the work and making radiologists more efficient, Hashim said, the company averages under 30 minutes for turnaround on reading a scan. And the ability for radiologists to do more reads means that productive doctors can get paid more. One obstacle to growth, he said, is that people still hold on to this conception that we need to have radiologists physically located in hospitals, he said.

Dr. Arthur Radow, a Phoenix-area radiologist and past president of the Arizona Radiological Society, said his radiology group joined Imaging Advantage in 2011. Under Imaging Advantage, the group has dropped from 18 doctors to about 12, and the remaining doctors are 60% to 80% more productive and are making more money, he said.

Midland College is committed to creating an educated workforce by offering occupational programs that lead to gainful employment.

Health Data Specialist – Physician Practice

The Health Information Management online associate degree program is accredited by the Commission on Accreditation for Health Informatics and Information Management Education (CAHIIM). According to the Bureau of Labor Statistics, health information management jobs are expected to increase by 15% from 2014-2024. The median income is $37,110, (2015 statistic), but additional credentials do add to the marketability of professionals.

The HIM program was established in 1998 as a traditional face-to-face program. In 2007-2008, the HIM program transitioned to an “online only” program to meet the needs of working students and professionals. The exception to online courses are the requirement for clinical courses completed onsite at a facility (HITT 2261 for the associate degree and HITT 1167 for Medical Coding Specialist certificate).

Many students admitted into the program have already completed one degree, and some have bachelors or masters degrees. The reason they pursue the associate degree training is to be eligible for the Registered Health Information Technician (RHIT) upon graduation. The RHIT credential, and the RHIA credential for the bachelor’s degree, continue to be a preferred credential that healthcare employers seek when hiring Health Information Management candidates.

Students must be self-motivated, able to comprehend information in written form, and have the ability to communicate effectively both in written and verbal forms. Students should also be able to research websites, upload and download documents, post discussions, and access email. The courses are not self-paced, but have scheduled weekly due dates for assignments, discussions, and tests. Students should expect to spend 6 9 hours per 3 hour credit course to complete assignments, readings, and tests. Instructors are available to answer questions online or by phone related to course topics.

Skills Needed for Health Information Profession:

ability to analyze patient data for payment or registry information,
able to sit for long periods to complete work at work station
research and review patient information for regulatory compliance,
code and categorize patient information using standard coding systems.
communicate effectively with patients, physicians, and other customers
ability to work under pressure to meet deadlines
keyboarding skills and ability to use software systems, patient portals, etc.

Students may apply to the program in the semester they are completing the prerequisites. Final grades will be verified before admittance. Students are accepted prior to each semester (Summer, Fall, and Spring). Most of the students in the programs are considered part-time; taking 2 to 3 classes each semester since most have part-time or full-time jobs. There are approximately 70-80 students taking HIM classes or prerequisite courses to gain admission to the program.

Clinical Requirement Onsite:

Students that live outside the local Midland area are able to locate clinical sites in their area with the assistance of the HIM program. More information and directions are given to students upon acceptance into the program (courses related to associate degree and Medical Coding Specialist certificate). Prior to the clinical course, students are required to meet certain facility requirements to attend clinical practice. These include a background check, drug screen, immunizations, and other requirements based of the facility. More information provided prior to clinicals.

Automates the process of scheduling patient visits. Features include automated follow-ups, text message/phone/email reminders and multi-location support. Typically offered with billing in a practice management suite.

Manages the operations and workflow of radiology imaging centers. Automates the process of storing, manipulating and distributing patient data and images.

Manages the storage and retrieval of DICOM images (X-rays, CAT scans, MRIs etc.). Often used in conjunction with an RIS to execute the radiology workflow efficiently.

What Type of Buyer Are You?

Most organizations we speak with are researching and evaluating medical software for one or more of the following reasons:

Transitioning from paper charts to digital records. It s raining paper is the common cry we hear from paper-based practices. These buyers want to cut back on paper, improve office efficiency, reduce errors and run a more effective operation overall.

Replacing outdated software. This is a common scenario we hear from buyers. Their current system whether it be a homegrown system or from a medical software vendor is out of date and costly to maintain or update. They want a more modern system that is easier to use, meets federal requirements (e.g. ONC-ATCB certification) or that meets feature/functional needs.

Combining applications into an integrated suite. In many cases these practices have a hodgepodge of disparate applications, and as a result, find themselves doing double data entry and dealing with other inefficient workflows and processes. These organizations invest in integrated medical office management software that is, integrated EMR, billing and scheduling applications to centralize all information and functions in one place.

Implementing best-of-breed applications. Conversely, these buyers are focused on applications to address a specific need. Most often, buyers in this category are looking for a stand-alone billing, EMR, RIS or PACS system.

Pursuing federal incentives. We re three years into the HITECH Act, and many physicians are replacing their EHR or purchasing one for the first time to meet federal requirements. In order to qualify for Medicare and Medicaid incentives, physicians or more accurately, eligible professionals must make meaningful use of a certified EHR. Providers have until 2015 to qualify, or they face penalties in the form of decreased reimbursements.

We should note that outpatient and inpatient organizations often have different feature/functional requirements. For example, inpatient care provider centers such as hospitals will require systems to support bed management, UB-04 billing and potentially long-term patient stays. Meanwhile, ambulatory care providers such as family medicine physicians and specialists will share common feature requirements to support walk-in/walk-out care.

Benefits of Medical Software

The general benefits of any medical system are improved quality of patient care, increased operational efficiency and improved practice profitability. These benefits are created by different applications and impact organizations in different ways.

  • The automation of back-office operations streamlines administrative tasks associated with patient encounters, which may enable providers to spend more time with patients and hire fewer staff.
  • More accurate documentation of these encounters and a more organized claims submission process can lead to improved collections.
  • Automated alerts prompt providers with potential issues or risks, while automated reminders help patients return to the office when necessary, improving quality of care.

In addition to these general benefits, the major applications found in medical software each provide a host of specific benefits. For example, in 2014 we surveyed physicians about the benefits of electronic health records. They cited easy access to records, more robust/legible records and drug interaction alerts as the top advantages of using an EHR.

Important Considerations

Integrated suite vs. best-of-breed. When selecting a system, buyers will have the choice of implementing different applications for specific tasks, or a complete suite of tools to address all their needs. The key decision that most providers will need to make is whether to implement a standalone electronic medical records (EMR) system or replace an existing practice management system with a complete system. We hear from many buyers facing this decision as practice management systems have been ubiquitous since the 1990s and EMRs are increasing in adoption, primarily due to the HITECH Act.

Software-as-a-Service (SaaS). The trend toward cloud computing is impacting many industries, and healthcare is certainly one of them. Web-based, or SaaS, software offers several advantages such as lower upfront costs, reduced IT and support costs, remote accessibility and more. However, practices in rural settings may not have access to the broadband Internet necessary to efficiently run Web-based software. Moreover, Web-based systems may not support all the feature/functional needs of some practices with unique requirements.

Mobile computing. Going hand-in-hand with SaaS, healthcare providers are finding themselves increasingly on the go and accessing systems from multiple offices, home and mobile devices. Tablet (e.g. iPad) and smartphone support, including iPhones and Android phones, is increasingly common. If you will be accessing your software primarily from a mobile device, we suggest choosing a vendor that has developed a native app for your device, such as MediTouch s iPad EMR.

ONC-ATCB certification. As most healthcare professionals are aware, the HITECH Act of 2009 requires the use of electronic medical records systems by 2015. Eligible professionals can subsequently qualify for up to $44,000 through the Medicare EHR Incentive Program or up to $63,750 through the Medicaid EHR Incentive Program. To qualify, they will need to demonstrate meaningful use of one of the ONC-ATCB certified EMRs .

Recent Events You Should Know About

Epic partners with BI software vendor Tableau. In February 2016, Tableau software announced a technology agreement with Epic. This deal gives Epic s medical software users greater access to analytics tools. For example, any dashboards created on Tableau will be integrated in Epic s EHR.

Vendor Aprima acquires EHR reseller Healthcare Data Solutions. The deal, which was announced in January 2017, means Aprima will now service Healthcare Data Solutions customer base of 300 providers in 125 practices across the U.S.

McKesson plans to buy CoverMyMeds. In January 2017, McKesson Corp. announced it will be paying $1.1 billion for CoverMyMeds, a company that automates the process of prior authorization for pharmacies, providers, drugmakers and payers. The deal is expected to close before July 2018. CoverMyMeds is expected to keep operating as an independent business unit under its current leadership team.
